VAAFT结合改良LIFT微创治疗ParksⅡ型肛瘘临床分析

摘　　要：为观察视频辅助肛瘘治疗(VAAFT)结合改良括约肌间瘘管结扎术(改良LIFT)微创治疗ParksⅡ型肛瘘的临床疗效,回顾性分析我院肛肠外科在2017年6月至2019年9月采用VAAFT结合改良LIFT微创治疗的32例ParksⅡ型肛瘘患者临床资料。结果显示,30例患者均一期愈合,2例患者术后残留外括约肌段瘘管内积脓,经换药后二期愈合。32例患者术后外括约肌均未切断或切开,并且术后行肛门直肠测压,肛管静息压正常,患者自述肛门精细控便功能与术前无明显差异。结果表明,改良LIFT是VAAFT微创手术中处理内口的好方法,是保证手术后不复发的一个重要因素。In order to investigate the clinical effect of ParksⅡanal fistula mini-invasitively treated with video-assisted anal fistula treatment(VAAFT)plus ligation of intersphincteric fistula tract(LIFT),authors reviewed the data of 32anal fistula patients received above procedure combined from 2017-06to 2019-09in author's hospital(coloproctological surgery department).As results,30cases were primarily heald with wound,rest 2cases were secondaryly healing through dressing exchange due to postoperative residual effusion in fistula tract within external sphincter segment;all 32patients'external sphincter were all not cut or ampulated after surgery,and their postoperative ano-rectal pressure,and anal resting pressure were all normal,as well as the patients-described finely controlled fecel function had no distinct difference than before surgery.Results show that modified LIFT is better method for managing inner-outlet amidst VAAFT,and a important factors to avoid postoperative recurrence.
