生脉胶囊对老年慢性心力衰竭患者心室重构和心肌纤维化的影响

摘　　要：目的观察生脉胶囊对老年慢性心力衰竭(心衰)患者心室重构和心肌纤维化的影响。方法将82例老年慢性心衰患者随机均分为2组:对照组(n=41)给予常规西医治疗,观察组(n=41)在对照组治疗的基础上加以生脉胶囊治疗。对比2组治疗疗效,统计2组患者治疗前后慢性心功能不全QOL量表(MLHFQ)评分、6 min步行距离、心室重构指标[左室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、左室收缩末期内径(LVESD)和左心室收缩末期容积(LVESV)]、心肌纤维化指标[血清结缔组织生长因子(CTGF)、基质金属蛋白酶-2(MMP-2)、转化生长因子β1(TGF-β1)、透明质酸(HA)、Ⅰ型前胶原(PCⅠ)、层黏连蛋白(LN)和Ⅲ型前胶原(PCⅢ)]、血清炎症因子[肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)]水平。结果观察组治疗总有效率(95.12%)显著高于对照组(80.49%)(P<0.05)。与治疗前相比,2组治疗后的MLHFQ评分均显著降低(P<0.05),6 min步行距离显著延长,且观察组治疗后的MLHFQ评分显著低于对照组,6 min步行距离显著长于对照组(P<0.05)。观察组治疗后的LVEDD、LVESD和LVESV均显著低于对照组(P<0.05),LVEF显著高于对照组(P<0.05)。2组治疗后的血清CTGF、MMP-2、TGF-β1、HA、PCⅠ、LN、PCⅢ、TNF-α和IL-1β水平均较治疗前显著降低(P<0.05),且观察组显著低于对照组(P<0.05)。结论生脉胶囊治疗老年慢性心衰疗效满意,可显著改善患者的生活质量,改善心室重构指标,延缓心肌纤维化。Objective To observe the effect of Shengmai Capsules on ventricular remodeling and myocardial fibrosis in elderly patients with chronic heart failure.Methods 82 elderly patients with chronic heart failure were randomly divided into 2 groups:the control group(n=41)were treated with conventional western medicine,and the observation group(n=41)was treated with Shengmai Capsules on the basis of control group treatment.The therapeutic effects of the 2 groups were compared,including the patients′chronic cardiac insufficiency QOL scale(MLHFQ)scores,6 minutes walking distance,and ventricular remodeling indexes[left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),left ventricular end systolic volume(LVESV)],myocardial fibrosis index[connective tissue growth factor(CTGF),matrix metalloproteinase(MMP-2),transforming growth factorβ1(TGF-β1),hyaluronic acid(HA),typeⅠprocollagen(PCⅠ),laminin(LN)and typeⅢprocollagen(PCⅢ)],serum inflammatory factors[tumor necrosis factor-α(TNF-α)and interleukin-β(IL-1β)]levels.Results The total effective rate of the treatment in the observation group was significantly higher than that in the control group(95.12%vs 80.49%)(P<0.05).Compared with before treatment,the MLHFQ scores of the 2 groups after treatment were significantly reduced(P<0.05),the 6 minutes walking distance was significantly extended,and the MLHFQ score of the observation group after treatment was significantly lower than that of the control group,and the 6 minutes walking distance was significantly longer than the control group(P<0.05).The LVEDD,LVESD and LVESV of the observation group after treatment were significantly lower than those of the control group(P<0.05),and the LVEF was significantly higher than that of the control group(P<0.05).The serum CTGF,MMP-2,TGF-β1,HA,PCⅠ,LN,PCⅢ,TNF-αand IL-1βlevels after treatment in the 2 groups were significantly lower than before treatment(P<0.05),and the observation group was significa
