PDCA循环法用于碳青霉烯类抗菌药物多学科协作管理项目的效果分析

摘　　要：目的提高碳青霉烯类抗菌药物的合理应用。方法基于由医务科牵头,药剂科、医院感染管理办公室、信息科、微生物检验科及临床科室共同组成的多学科协作管理体系,采用PDCA循环法优化碳青霉烯类抗菌药物的管理体系,比较PDCA循环法实施前(2019年4月至12月,1715份)和实施后(2020年4月至12月,788份)碳青霉烯类抗菌药物的使用情况。结果与PDCA循环法实施前比较,实施后的碳青霉烯类抗菌药物使用率、使用强度、费用占总药品费用比例、费用占总抗菌药物费用比例均显著下降,且处方合理率显著提升(P<0.01)。结论PDCA循环法可优化多学科协作管理项目,促进临床合理使用碳青霉烯类抗菌药物,但仍需进一步完善相关措施。Objective To improve the rational application of carbapenems.Methods Based on the multidisciplinary collaborative management system led by the medical department and composed of the pharmacy department,nosocomial infection management office,information department,microbiological laboratory and clinical departments,the management system of carbapenems was optimized by PDCA cycle,and the application data of carbapenems was compared before the implementation of PDCA cycle(1715 prescriptions,from April to December 2019)and after the implementation(788 prescriptions,from April to December 2020).Results Compared with those before the implementation of PDCA cycle,the utilization rate,antibiotics use density(AUD),the proportion of consumption sum of carbapenems to total consumption sum of drugs,the proportion of consumption sum of carbapenems to total consumption sum of antibiotics decreased significantly,while the rationality rate of prescription increased significantly(P<0.01).Conclusion PDCA cycle can optimize multidisciplinary collaborative management and promote the rational use of carbapenems in the clinic,but the relevant measures still need to be further improved.
