双黄液熏洗坐浴联合一次性根治术治疗低位肛周脓肿临床评价

摘　　要：目的探讨中药自拟方双黄液熏洗坐浴联合一次性根治术治疗低位肛周脓肿的疗效,以及对术后创面愈合的影响。方法选取鄂东医疗集团黄石市中医医院肛肠科2020年7月至10月收治的低位肛周脓肿患者70例,按就诊顺序随机分为对照组和观察组,各35例。两组患者均行一次性根治术,并于术后给予常规抗炎及对症治疗,对照组在此基础上予高锰酸钾溶液熏洗坐浴,观察组予自拟方双黄液熏洗坐浴。结果观察组和对照组总有效率均为100.00%(P>0.05);与对照组比较,观察组术后3,7 d的视觉模拟评分量表(VAS)评分均显著降低,创面渗出评分均显著升高(P<0.05);两组肉芽组织生长评分差异不显著(P>0.05);观察组腐肉脱落时间及创面愈合时间均显著短于对照组,术后21 d创面愈合率显著高于对照组(P<0.05)。结论双黄液熏洗坐浴联合一次性根治术治疗低位肛周脓肿疗效较好,可有效缩短术后创面的愈合时间,减轻疼痛感,减少创面渗出,促进术后恢复。Objective To investigate the clinical efficacy of Shuanghuang Liquid fumigation and hip bath combined with one-time radical operation in the treatment of low perianal abscess and its effect on postoperative wound healing.Methods A total of 70 patients with low perianal abscess admitted to the Huangshi Traditional Chinese Medicine Hospital of Edong Healthcare Group from July to October 2020 were selected and randomly divided into the control group and the observation group according to the order of visit,35 cases in each group.The patients in the two groups underwent one-time radical operation,and they were given routine anti-inflammatory,dressing change and symptomatic treatment after the operation.On this basis,the patients in the control group were given fumigation with potassium permanganate solution,while the patients in the observation group were given fumigation and hip bath with self-made Shuanghuang Liquid.Results The total effective rates of the observation group and the control group were 100.00%(P>0.05).Compared with those in the control group,the visual analogue scale(VAS)score in the observation group decreased significantly and the wound exudation score increased significantly on the third and seventh day after the operation(P<0.05).There was no significant difference in the granulation tissue growth score between the two groups(P>0.05).The carrion shedding time and wound healing time in the observation group were significantly shorter than those in the control group,and the wound healing rate in the observation group was significantly higher than that in the control group on the 21 st day after the operation(P<0.05).Conclusion Shuanghuang Liquid fumigation and hip bathing combined with one-time radical operation is effective in the treatment of low perianal abscess,which can effectively shorten the postoperative wound healing time,reduce pain and wound exudation,and promote postoperative recovery.
