针刀治疗腰椎间盘突出症所致坐骨神经痛临床研究

摘　　要：目的探究针刀治疗腰椎间盘突出症所致坐骨神经痛的临床效果。方法选择2018年7月—2020年2月于南昌市洪都中医院接受治疗的140例腰椎间盘突出症所致坐骨神经痛患者作为研究对象,随机分为传统组和针刀组,每组70例。采用推拿+针刺治疗的患者列入传统组,采用针刀治疗的患者列入针刀组。比较2组患者的视觉疼痛模拟量表(Visual analogue scale,VAS)评分和步行能力。结果治疗前,2组的VAS评分比较,差异无统计学意义(P>0.05);针刀组治疗后的VAS评分低于传统组,差异有统计学意义(P<0.05)。针刀组的步伐速度、步伐频率快于传统组,步伐长度长于传统组,步伐幅度大于传统组,患肢单支撑相、双支撑相和摆动相高于传统组,差异均有统计学意义(P<0.05)。结论针刀在腰椎间盘突出症所致坐骨神经痛康复治疗中的效果显著,能缓解患者疼痛程度,改善步行能力,值得临床推广。Objective To explore the clinical effect of acupotomy in the treatment of sciatica caused by lumbar disc herniation.Methods A total of 140 patients with sciatica caused by lumbar disc herniation who received treatment in Nanchang Hongdu Hospital of Traditional Chinese Medicine from July 2018 to February 2020 were retrospectively selected as study subjects.They were divided into the traditional group and the acupotomy group,with 70 patients in each group.Patients treated with massage and acupuncture was included in the traditional group,while those treated with acupotomy were included in the acupotomy group.The visual analogue scale(VAS)score and walking ability were compared between the two groups.Results Before treatment,there was no significant difference in VAS score between the two groups(P>0.05).VAS score of the acupotomology group was lower than that of the traditional group after treatment,and the difference was statistically significant(P<0.05).The pace speed and pace frequency of the acupotomology group were faster than that of the traditional group,the pace length was longer than that of the traditional group,the pace amplitude was greater than that of the traditional group,and the single support phase,double support phase and swing phase of the affected limb were higher than that of the traditional group,with statistical significance(P<0.05).Conclusion Acupotomy is effective in the rehabilitation treatment of sciatica caused by lumbar disc herniation.It can relieve the pain degree of patients and improve their walking ability.It is worthy of clinical promotion.
