摘　　要：目的观察眶隔-睑板与眼轮匝肌-睑板双重固定的重睑切开术应用于上睑臃肿患者的效果。方法回顾性分析53名上睑臃肿的单睑患者,在去除部分肥厚的眼轮匝肌后脂肪、眶隔脂肪及增厚的眶隔后,采用眶隔-睑板和眼轮匝肌-睑板的双重内固定方式,达到去除患者上睑多余组织、恢复患者眶隔止点位置的目的。手术切口采用切口下缘皮肤眼轮匝肌-睑板-切口上缘皮肤眼轮匝肌缝合关闭。结果本组53例患者(共106侧),术后随访6~24个月,平均8.7个月。所有患者手术切口均一期愈合,无切口感染及瘢痕增生发生。53例患者中3例重睑变浅,0例复发。将本方法应用于上睑臃肿患者后,单睑复发率明显低于不加内固定的重睑术和Park法重睑术(P<0.05)。结论对于上睑臃肿的单睑患者,尤其是东亚人群,采用眶隔-睑板与眼轮匝肌-睑板双重固定的重睑切开术可获得更持久稳定的重睑形态,单睑复发率明显降低,值得临床推广应用。Objective To observe the efficacy of upper blepharoplasty using double fixation technique of orbital septum-tarsal plate and orbicularis oculi muscle-tarsal plate in patients with puffy upper eyelids.Methods The total of 53 patients with puffy upper eyelids were included in this study.After excessive retro-orbicularis fat and orbital fat resection and partial thickened orbital septum removal,all patients underwent double fixation technique for removing excessive upper eyelid tissue and restoring position of orbital septum.The surgical incision was closed by inferior skin and orbicularis oculi muscle-tarsal plate-superior skin and orbicularis oculi muscle suture.Results All 53 patients(106 eyes)were followed up for 6-7 months(mean 8.7 months).All the incisions got primary healing without infection or scar hypertrophy.Among 53 patients,the skin fold in 3 patients became shallow but no patient relapsed.Base on our previous work,the recurrence rate of double fixation group was significantly lower than that of the conventional method group and Park method group(P<0.05).Conclusion For puffy single eyelid patients,especially for Asian people,modified blepharoplasty combined with double fixation technique of orbital septum-tarsal plate and orbicularis oculi muscle-tarsal plate can obtain more durable and stable double eyelid morphology and significantly reduce the recurrence rate of single eyelid,which is worthy of clinical application.
