微创钥匙孔负压抽吸术治疗腋臭疗效观察及术后并发症影响因素分析

摘　　要：目的探讨微创钥匙孔负压抽吸术治疗腋臭的疗效,以及与术后并发症相关的影响因素。方法2018年5月至2019年12月,56例腋臭患者根据手术类型分为观察组(n=36)和对照组(n=20)。观察组患者予以微创钥匙孔负压抽吸术进行治疗,对照组患者给予小切口皮下剪除手术进行治疗。比较两组患者的临床疗效、手术时间、伤口愈合时间及并发症情况,记录并分析与微创钥匙孔负压抽吸术后并发症相关的影响因素。结果与对照组相比,观察组患者临床总有效率高(P<0.05),手术时间和伤口愈合时间短(P<0.05),并发症发生率低(P<0.05)。男性、肥胖、Park&Shin分级为3级、不遵医嘱是导致微创钥匙孔负压抽吸术后并发症发生的影响因素(P<0.05)。结论微创钥匙孔负压抽吸术与小切口皮下剪除手术治疗腋臭相比,能够显著缩短手术时间和伤口愈合时间,降低术后并发症发生率,具有更好的临床疗效;男性、肥胖、Park&Shin分级为3级、不遵医嘱是微创钥匙孔负压抽吸术后并发症的重要危险因素,需引起临床重视并提前干预。Objective To explore the effects of minimally invasive keyhole vacuum suction in the treatment of axillary bromhidrosis and the influencing factors of postoperative complications.Methods From May 2018 to December 2019,56 patients with axillary bromhidrosis were selected and were divided into the observation group(n=36)and control group(n=20)according to the type of operation.The patients in the observation group were treated with minimally invasive keyhole vacuum suction,and the patients in the control group were treated with small incision subcutaneous excision.The clinical effects,operation time,wound healing time and the occurrence of complications between the two groups were compared,and the influencing factors of postoperative complications were recorded and analyzed.Results Compared with the control group,the total effective rate in the observation group was higher(P<0.05),the operation time and wound healing time in the observation group were shorter(P<0.05),and the incidence of complications in the observation group was lower(P<0.05).The incidence of complications of minimally invasive keyhole vacuum suction was higher in male,obesity,Park&Shin grade 3 and non-compliant patients(P<0.05).Conclusion In the treatment of axillary bromhidrosis,compared with small incision subcutaneous excision,minimally invasive keyhole vacuum suction can significantly shorten the operation time and wound healing time,and reduce the incidence of postoperative complications,and have higher clinical effects.Male,obesity,Park&Shin grade 3,non-compliance are important risk factors for complications after minimally invasive keyhole vacuum suction,which need clinical attention and early intervention.
