慢性乙型肝炎患者HIF-1α水平与肝纤维化相关性探讨

摘　　要：目的探究与分析慢性乙型肝炎患者中的外周血缺氧诱导因子-1α(hypoxia-inducible factor 1-alpha,HIF-1α)水平与肝纤维化状态的相关性。方法选取医院2019年5月—2021年5月收治的慢性乙型肝炎患者96例作为观察组,全部患者均接受了肝穿刺活检,并结合其病理结果判断患者的肝纤维化分期,参考肝纤维化分期标准将患者分为S1组(n=25)、S2组(n=22)、S3组(n=28)、S4组(n=21)。测量外周血中的HIF-1α水平及肝纤维化血清学标志物,并结合其检测结果进行统计学分析。结果S2组、S3组、S4组分别与S1组相比,S3组、S4组分别与S2组相比,S4组与S3组相比,HIF-1α、HA、PⅢNP、C-Ⅳ、LN水平较高,差异有统计学意义(P<0.05)。经过Spearman分析可见,外周血中的HIF-1α分别与HIF-1α、透明质酸(hyaluronic acid,HA)、Ⅲ型前胶原氨端肽(precollagenⅢN-terminal peptide,PⅢNP)、Ⅳ型胶原(collage typeⅣ,C-Ⅳ)及层黏连蛋白(laminin,LN)水平呈现出明显的正相关性(P<0.05)。结论慢性乙型肝炎患者中的外周血HIF-1α水平与肝纤维化处于的阶段密切相关,该病指标的变化在预测肝硬化上具有较高的早期应用价值。Objective To explore and analyze the correlation between the level of peripheral blood hypoxia-inducible factor-1α(HIF-1α)and liver fibrosis in patients with chronic hepatitis B.Methods Ninety-six patients with chronic hepatitis B who were admitted to our hospital from May 2019 to May 2021 were selected as the observation group.All patients were received liver biopsy,and the pathological results were used to determine the stage of liver fibrosis.According to the liver fibrosis staging standard,patients were divided into S1 group(n=25),S2 group(n=22),S3 group(n=28),S4 group(n=21).Measured the level of HIF-1αin the peripheral blood and the serum markers of liver fibrosis,and combine the detection results for statistical analysis.Results S2,S3,S4 were compared with S1,S3,S4 were compared with S2,S4 was compared with S3,HIF-1α,HA,PⅢNP,C-Ⅳ,LN levels were higher,the difference was statistically significant(P<0.05).Spearman analysis showed that HIF-1αin peripheral blood was significantly positively correlated with HIF-1α,HA,PⅢNP,C-Ⅳ,and LN levels respectively(P<0.05).Conclusion Peripheral blood HIF-1αlevel in patients with chronic hepatitis B is closely related to the stage of liver fibrosis,and changes in disease indicators have a high early application value in predicting liver cirrhosis.
