摘　　要：目的观察来氟米特治疗红皮病型银屑病(EP)的临床效果及对外周血肿瘤坏死因子(TNF)-α及白细胞介素(IL-17)的影响。方法选取2016年1月—2018年12月25例轻型EP住院患者分为两组,分别给予复方甘草酸苷联合阿维A胶囊及复方甘草酸苷联合来氟米特治疗,对比分析治疗前后的银屑病PASI评分,同时采用双抗体夹心酶联免疫吸附法(ELISA)分别检测EP患者治疗前后血清TNF-α及IL-17的水平。结果两组EP患者治疗后PASI评分均明显下降,两组间比较治疗有效率(对照组76.92%,试验组66.67%)和住院平均日[对照组(17.26±3.93)d,试验组(18.19±4.14)d]差异无统计学意义(χ2=0.326,P>0.05;t=0.563,P>0.05);两组治疗后TNF-α及IL-17水平均明显下降,两组间比较试验组TNF-α水平[对照组治疗后(98.35±15.92)pg/mL,试验组治疗后(81.35±11.31)pg/mL]较低,差异有统计学意义(t=3.011,P<0.05),IL-17下降,差异无统计学意义(t=0.682,P>0.05)。结论来氟米特是治疗红皮病型银屑病的一个可供选择、安全有效药物,可降低血清TNF-α、IL-17水平,TNF-α水平降低明显。Objective To observe the treatment of leflunomide on EP and effect on TNF-αand IL-17.Methods From January 2016 to December 2018,25 hospitalized patients with mild EP were divided into two groups.They were respectively given glycyrrhizin combined with Retinoic acid and glycyrrhizin combined with leflunomide.The PASI scores were analyzed,and serum levels of TNF-αand IL-17 were detected by ELISA before and after treatment.Results PASI scores of the 2 groups reduced after treatment.There was no difference in efficiency(control group 76.92%,experimental test group 66.67%,χ2=0.326,P>0.05)and average hospital day[control group(17.26±3.93)d,experimental group(18.19±4.14)d,t=0.563,P>0.05];the levels of TNF-αand IL-17 were significantly reduced after treatment.Compared 2 groups,the experimental group was lower in TNF-α[experimental group after treatment(98.35±15.92)pg/mL,control group after treatment(81.35±11.31)pg/mL](t=3.011,P<0.05),not in IL-17(t=0.682,P>0.05).Conclusion Leflunomide is an alternative,safe and effective drug for the EP treatment.It can decrease TNF-αand IL-17 in serum,especially the TNF-α.
