摘　　要：目的探讨与分析TTM模型下饮食和运动干预在妊娠期糖尿病患者中的应用。方法采取随机数字表法对我院自2018年5月—2020年5月收治的妊娠期糖尿病患者90例分组,各45例,对照组给予常规干预,观察组给予行为阶段转变理论模型(the transtheoretical model and stages of change,TTM)下饮食和运动干预,对比两组疾病相关知识掌握情况、实验室指标、妊娠及新生儿结局。结果观察组与对照组相比患者基础知识、母婴危害知识、饮食及运动知识、血糖监测知识、体质量管理知识掌握情况均更好,空腹血糖及餐后2 h血糖值相比均较低,剖宫产率、早产率、妊娠高血压率、胎膜早破率及羊水过多率均较低,新生儿Apgar评分较高、新生儿出生后体质量较低、新生儿低血糖或感染性疾病率均较低,差异有统计学意义(P<0.05)。结论TTM模型下饮食和运动干预提高了妊娠期糖尿病患者对健康相关知识的掌握情况,改善了血糖指标,避免获得不良妊娠结局。Objective To explore and analyze the application of diet and exercise intervention in patients with gestational diabetes under the TTM model.Methods A random number table was used to divide 90 patients with gestational diabetes who were admitted to our hospital from May 2018 to May 2020,with 45 cases in each group.The control group was given routine intervention,and the observation group was given diet and exercise intervention under the TTM model.Compare the two groups of disease-related knowledge mastery,laboratory indicators,pregnancy and neonatal outcomes.Results Compared with the control group,the observation group had better grasp of basic knowledge,maternal and infant hazard knowledge,diet and exercise knowledge,blood glucose monitoring knowledge,and body mass management knowledge.The fasting blood glucose and blood glucose levels after a meal were both lower than those in the control group.Cesarean section rate,premature delivery rate,pregnancy-induced hypertension rate,premature rupture rate of membranes and hyperamniotic fluid rate are all low,neonatal Apgar score is higher,neonatal weight is lower after birth,neonatal hypoglycemia or infection the rates of sexual diseases were low,and the difference was statistically significant(P<0.05).Conclusion Diet and exercise intervention under the TTM model improves the knowledge of health-related knowledge in patients with gestational diabetes,improves blood glucose indicators,and avoids adverse pregnancy outcomes.
