摘 要:目的探讨2型糖尿病(T2DM)患者血清非高密度脂蛋白胆固醇(non-high density lipoprotein cholesterol,nonHDL-C)、胱抑素C(cystatinC,CysC)与视网膜病变发病风险的相关性。方法收集144例T2DM患者的临床资料,根据眼底拍照结果分成视网膜病变组(DR组)和单纯糖尿病组(DM组),比较两组non-HDL-C、CysC等指标的差异并分析其与DR的相关性。结果 DR组患者病程、CysC、TC、HDL-C、LDL-C、ApoA、ApoB、HbA1c和non-HDL-C与DM组比较,差异有统计学意义(P <0.05);Pearson相关分析结果显示DR分级与CysC、TC、HDL-C、LDL-C、non-HDL-C、ApoA、ApoB、HbA1c呈正相关(P <0.01);多元Logistic回归分析显示,CysC、HbA1c、ApoA、病程为DR的影响因素(P <0.05)。结论 nonHDL-C、CysC、HbA1c对DR诊断有重要价值,联合检测nonHDL-C、CysC、HbA1c等指标,对于DR的早期风险评估有重要意义。Objective To investigate the correlation between non-HDL-C,CysC and the risk of diabetes retinopathy(DR)in type 2 diabetes mellitus.Methods The clinical data of 144 patients with T2DM were collected and divided into DR group and DM group according to the fundus photo results.The differences of blood lipid indexes such as CysC,HbA1c and non-HDL-C in the two groups were compared and the correlation between them and DR was analyzed.Results The course of disease,CysC,TC,HDL-C,LDL-C,ApoA,ApoB,HbA1c and non-HDL-C in DR group were significantly different from those in DM group(P<0.05).Pearson correlation analysis showed that DR grading was positively correlated with CysC,TC,HDL-C,LDL-C,non-HDL-C,ApoA,ApoB,and HbA1c(P<0.01).Multivariate logistic regression analysis showed that CysC,HbA1c,ApoA,and the course of the disease were the influencing factors of DR(P<0.05).Conclusion non-HDL-C,CysC and HbA1c are of great value in the diagnosis of DR.Combined detection of non-HDL-C,CysC,HbA1c and other indicators is of great significance for the early risk assessment of DR.
