某院癌症患者阿片类药物应用合理性评价

摘　　要：目的观察泗洪县人民医院癌症患者阿片类药物应用现状,并评价用药合理性。方法回顾性分析2019年1月~2020年1月泗洪县人民医院收治的52例服用阿片类药物的癌症患者的临床资料。观察各种阿片类药物使用情况,评价阿片类药物应用合理性。结果52例服用阿片类药物的癌症患者共应用4种剂型的阿片类药物,其中缓释片总用量占比最高,为77.56%,其次为贴剂、片剂、注射剂。阿片类整体合理用药占比为93.98%,吗啡注射液合理用药占比最低(83.33%),不合理用药中,爆发痛处置不合理占比最高为60.00%,其次为用药频率和用药剂量不合理。阿片类药物使用频率(DDDs)排名中,芬太尼透皮贴剂、羟考酮缓释片用药频率均较高。结论癌症患者使用阿片类药物整体合理,但临床工作者仍需密切关注用药剂量、用药频率、爆发痛处置不合理问题。Objective To observe the current status of opioid use in cancer patients in Sihong County People's Hospital,and to evaluate the rationality of the medication.Methods A retrospective analysis of the clinical data of 52 cancer patients taking opioids admitted to Sihong County People's Hospital from January 2019 to January 2020.The status quo of the application of various opioids were observed,and the rationality of the application of opioids were evaluated.Results 52 cancer patients in our hospital used 4 dosage forms of opioids,of which the total dosage of sustained-release tablets accounted for 77.56%,followed by patches,tablets,and injections.The overall rational use of opioids accounted for 93.98%,the rational use of morphine injection was the lowest(83.33%),and the unreasonable treatment of outbreak pain accounted for the highest(60.00%),followed by the frequency of medication and the unreasonable dose of medication.Among the 52 cancer patients in our hospital in the frequency of opioid use(DDDs),fentanyl transparent patch and oxycodone sustained-release tablets are used more frequently.Conclusion The use of opioids by cancer patients in our hospital is generally reasonable,but clinicians still need to pay close attention to the problems of medication dosage,medication frequency,and unreasonable management of outbreak pain.
