针刺配合雷火灸治疗脾肾两虚型多囊卵巢综合征的临床研究

摘　　要：目的观察针刺配合雷火灸治疗脾肾两虚型多囊卵巢综合征(PCOS)的临床疗效。方法将62例肥胖型PCOS患者随机分为对照组(31例,脱落2例)、治疗组(31例,脱落1例)。对照组给予氯米芬胶囊联合黄体酮胶丸治疗,治疗组在对照组的用药基础上给予针刺配合雷火灸治疗,疗程为3个月经周期,观察治疗前后患者体重指数(BMI)、血清性激素水平[黄体生成素(LH)、卵泡刺激素(FSH)、睾酮(T)]、糖脂代谢[空腹血糖(FPG)、空腹胰岛素(FINS)、脂联素(ADPN)、血清瘦素(LEP)]、卵巢一般情况。结果对照组总有效率68.97%,治疗组总有效率90.00%,治疗组疗效与对照组相比差异有统计学意义(P<0.05);治疗组治疗后患者基础BMI、LH、FINS、ADPN、LEP明显优于对照组,差异有统计学意义(P<0.01);而两组患者FSH、T、FPG、卵巢体积、窦状卵泡数较对照组差异均无统计学意义(P>0.05)。结论针刺配合雷火灸治疗可明显改善脾肾两虚型PCOS患者的内分泌糖脂代谢,是一种较好的治疗手段。Objective To observe the therapeutic effect of acupuncture and thunder-fire moxibustion on spleen and kidney deficiency-type polycystic ovary syndrome(PCOS).Methods A total of 62 patients with obesity-type PCOS were randomized into a control group(31 cases,2 cases dropped off)and a treatment group(31 cases,1 cases dropped off).The control group were treated by clomifene capsule combined with progesterone capsule,the treatment group,on the basis of the control group,acupuncture and thunder-fire moxibustion was applied.The course of treatment was 3 menstrual cycles.Before and after treatment,BMI,the gonadal hormone concentrations(LH,FSH,T),glycolipid metabolism(FPG,FINS,ADPN,LEP),general ovarian condition were observed and compared between the two groups.Results The total efficacy rate was 68.97%in the control group and 90.00%in the treatment group,the difference was statistically significant(P<0.05).In the treatment group,the basic BMI,LH,FINS,ADPN,LEP were apparently better than those in the control group,presenting the significant difference(P<0.01).In comparison of the two groups,the differences were not significant in basic FSH,T,FPG,ovarian volume and the number of antrum ovarian follicle(P>0.05).Conclusion Acupuncture and thunder-fire moxibustion therapy improves endocrinal and glycolipid metabolism level in PCOS patients,which suggests that as a good method is worthy of clinical application.
