卵圆孔未闭封堵器介入治疗房间隔缺损并房间隔膨出瘤患儿的效果分析

摘　　要：目的分析卵圆孔未闭封堵器介入治疗房间隔缺损并房间隔膨出瘤患儿的临床效果,总结治疗经验。方法选取2019年2月~2020年2月收治的34例房间隔缺损并房间隔膨出瘤患儿为研究对象,所有患儿均完善术前相关检查,选择合适的封堵器,行卵圆孔未闭封堵器介入治疗。分析患儿的手术情况,包括封堵器使用、手术时间、透视时间以及近期并发症发生情况等;随访6个月复查经胸超声,分析患儿的预后情况,并对比术前、术后的右心房前后径、右心房左右径、右心室内径变化。结果34例患儿均使用一枚封堵器封堵成功,封堵器直径(22.65±3.16)mm,手术时间(41.65±10.32)min,透视时间(3.93±1.54)min,封堵器释放后立即行经胸超声检查,房水无残余分流,透视观察封堵器形态良好,位置稳固、无移位,术中未发生并发症;所有患儿均随访6个月,随访率为100.00%,复查经胸超声提示封堵器位置稳固、无移位,形态良好,房水无残余分流,术后未发生并发症;患儿术后1 d以及1、3、6个月的右心房前后径、右心房左右径、右心室内径均小于术前,对比差异有统计学意义(P<0.05)。结论卵圆孔未闭封堵器介入治疗房间隔缺损并房间隔膨出瘤患儿的临床效果确切,安全性高,能明显改善患儿的经胸超声指标。Objective To analyze the clinical effect of transcatheter closure of patent foramen ovale in children with atrial septal defect(ASD)and atrial septal prolapse tumor.Methods 34 cases of atrial septal defect and atrial septal prolapse tumor in our hospital from February 2019 to February 2020 were selected as the research objects,all the children were improved the preoperative examination,and the appropriate occluder was selected for interventional treatment of patent foramen ovale occluder.The operation conditions of the children were analyzed,including the use of occluder,operation time,fluoroscopy time and recent complications;the patients were followed up for 6 months to review the transthoracic ultrasound,analyze the prognosis of the children,and compare the changes of right atrial anteroposterior diameter,right atrial left-right diameter and right ventricular diameter before and after the operation.Results The occluder diameter was(22.65±3.16)mm,the operation time was(41.65±10.32)min,and the fluoroscopy time was(3.93±1.54)min.Transthoracic ultrasound examination was performed immediately after the release of the occluder,and there was no residual shunt of aqueous humor.Fluoroscopy showed that the occluder was in good shape,stable position,no displacement,and no intraoperative complications.After 6 months,the follow-up rate was 100.00%.Transthoracic ultrasound showed that the occluder position was stable,no displacement,good shape,no residual shunt of aqueous humor,and no complications occurred after operation.The anteroposterior diameter of right atrium,left and right diameter of right atrium,and right ventricular diameter at 1 day,1,3,6 months after operation were less than those before operation,and the difference was statistically significant(P<0.05).Conclusion The clinical effect of transcatheter closure of patent foramen ovale occluder in the treatment of children with atrial septal defect and atrial septal prolapse tumor is exact and safe,which can significantly improve the transthoracic ultrasound in
