摘　　要：目的:评价由个案管理师主导的经导管主动脉瓣植入术(TAVI)围手术期管理方案的应用效果。方法:选取于2019年11月至2020年11月在我院行TAVI手术治疗的患者42例为研究对象,组建TAVI围手术期多学科管理团队、制定个案管理路径与干预方案,对研究对象开展个案管理,采用6分钟步行试验、Borg指数和健康调查12条简表(SF-12)测量患者术前、出院前、术后1个月、术后3个月的心脏功能和生活质量。结果:患者术后1个月、术后3个月6分钟步行距离较术前及出院前有所提升(P<0.05),心脏功能有所改善(P<0.05),术后生活质量较术前也有所提升(P<0.05)。结论:实施个案管理师主导的TAVI围手术期管理方案,有助于促进患者心脏功能恢复,提高患者生活质量。Objective:To explore the application of case manager-led TAVI perioperative management program in patients with aortic stenosis.Methods:Forty-two patients undergoing TAVI surgical treatment were selected from November 2019 to November 2020.Case management program was carried out by forming TAVI perioperative multidisciplinary team,case management path and intervention plan.Six Minute Walk Test(6MWT),Borg Scale and SF-12 were used to measure heart function and quality of life of patients before the surgery,before discharge,one month after and three months after the surgery.Results:Six Minute Walk Distance one month and three months after the surgery was significantly longer than that before discharge(P<0.05).There was statistical significance in terms of cardiac function in different time points(P<0.05).Quality of life of patients was significantly increased after the surgery compared with that before the surgery(P<0.05).Conclusion:The implementation of case manager-led TAVI perioperative management program is helpful to promote the recovery of cardiac function and improve the quality of life of patients.
