摘　　要：目的回顾性分析经后腹膜腹腔镜下去顶减压术治疗肾囊肿复发后的Bosniak分级情况。方法选取10例门诊随访的经后腹膜腹腔镜下肾囊肿去顶减压术后的复发患者,将手术前后Bosniak分级变化进行比较。结果10例患者手术前Bosniak分级分别为Ⅰ级8例,Ⅱ级1例,ⅡF级1例。患者于术后45天~5年复发,复发后影像学提示Ⅰ级5例,Ⅱ级4例,Ⅲ级1例。复发为Ⅲ级患者确诊为透明细胞性肾细胞癌。结论肾囊肿经后腹膜腹腔镜下去顶减压术治疗在复发后,Bosniak分级可能会改变,且存在恶变可能,必须定期随访。Objective To retrospectively analyze the Bosniak grading of recurrence renal cyst after retroperitoneal laparoscopic unroofing and decompression. Methods Ten patients with recurrent renal cyst after retroperitoneal laparoscopic unroofing and decompression were selected and the Bosniak grading changes before and after operation were compared. Results Among the ten patients,eight patients were in grade Ⅰ,one patient in grade Ⅱ,and one patient in grade ⅡF preoperatively. Recurrence occurred 45 days to 5 years postoperatively. There were five cases of grade Ⅰ,four cases of grade Ⅱ,and one case of grade Ⅲ in the images after recurrence. The patient with grade Ⅲ was diagnosed with clear cell renal cell carcinoma. Conclusions After recurrence,the Bosniak grade of renal cysts undergoing retroperitoneal laparoscopic unroofing and decompression may change. The recurrent renal cyst has a malignant tendency and regular follow-up is necessary.
