摘　　要：目的探讨有创-无创序贯机械通气治疗重症肺炎合并呼吸衰竭的效果及对患者血清SP-D、PARC/CCL18水平的影响。方法选择我院2018年12月至2019年12月收治的76例重症肺炎合并呼吸衰竭患者进行随机对照试验,使用随机数字表法将其分为有创组及序贯组,每组38例。有创组采用传统持续有创机械通气治疗,序贯组采用有创-无创序贯机械通气治疗。比较两组的治疗效果。结果治疗后,序贯组的TNF-α、IL-10、IL-6、IL-4、cTnⅠ、ANP、NT-proBNP、SP-D、PARC/CCL18水平显著优于有创组(P<0.05)。结论有创-无创序贯机械通气能有效减轻重症肺炎合并呼吸衰竭患者的炎症反应,下调血清SP-D、PARC/CCL18水平,保护心肌功能。Objective To investigate the effect of sequential invasive-noninvasive mechanical ventilation on severe pneumonia complicated with respiratory failure and its influences on serum SP-D and PARC/CCL18 levels in patients.Methods Seventy-six patients with severe pneumonia complicated with respiratory failure admitted in our hospital from December 2018 to December 2019 were selected for a randomized controlled trial.The patients were divided into invasive group and sequential group by random number table method,with 38 cases in each group.The invasive group was treated with traditional continuous invasive mechanical ventilation,while the sequential group was treated with sequential invasive-noninvasive mechanical ventilation.The treatment effects were compared between the two groups.Results After treatment,the levels of TNF-α,IL-10,IL-6,IL-4,cTnⅠ,ANP,NT-probNP,SP-D and PARC/CCL18 in the sequential group were significantly better than those in the invasive group(P<0.05).Conclusion Sequential invasive-noninvasive mechanical ventilation can effectively reduce the inflammatory response and the levels of serum SP-D and PARC/CCL18,and protect myocardial function in patients with severe pneumonia complicated with respiratory failure.
