实时超声造影对肝脏良恶性占位病变的诊断价值

摘　　要：目的探讨实时超声造影对肝脏良恶性占位病变的诊断价值。方法对2017年1月至2020年1月于我院就诊的120例肝脏占位性病变患者实施常规彩色多普勒超声检查、实时超声造影检查,参照手术病理诊断结果,比较两种检查方法的诊断效能。结果实时超声造影对肝癌的诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于常规彩色多普勒超声,差异具有统计学意义(P<0.05)。实时超声造影对直径<3、3~5 cm和Ⅰ、Ⅱ期肝癌的诊断符合率均高于常规彩超,差异具有统计学意义(P<0.05)。肝癌组的峰值强度高于肝脏良性占位病变组,增强峰值时间、开始增强时间、峰值消退时间均短于肝脏良性占位病变组,差异具有统计学意义(P<0.05)。结论实时超声造影可准确鉴别肝脏良恶性占位病变,对肝癌具有良好的诊断价值,值得推广。Objective To investigate the diagnostic value of real-time contrast-enhanced ultrasound in benign and malignant liver space occupying lesions.Methods A total of 120 patients with liver space occupying lesions treated in our hospital from January 2017 to January 2020 were examined by conventional color Doppler ultrasound and real-time contrast-enhanced ultrasound,and the diagnostic efficacy of the two methods was compared according to the surgical pathological diagnosis results.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of real-time contrast-enhanced ultrasound in the diagnosis of liver cancer were higher than those of conventional color Doppler ultrasound,and the differences were statistically significant(P<0.05).The diagnostic coincidence rates of real-time contrast-enhanced ultrasound in diameter<3,3-5 cm,and stagesⅠandⅡliver cancer were higher than those of conventional color Doppler ultrasound,and the differences were statistically significant(P<0.05).The peak intensity of the liver cancer group was higher than that of the benign liver space occupying lesion group,the peak enhancement time,start enhancement time and peak regression time were shorter than those of the liver benign space occupying lesion group,and the differences were statistically significant(P<0.05).Conclusion Real-time contrast-enhanced ultrasound can accurately differentiate benign and malignant liver space occupying lesions,and it has good diagnostic value for liver cancer,which is worthy of promotion.
