摘  要:目的比较研究脱细胞异体真皮及自体刃厚皮片修复外耳道皮肤缺损的治疗效果。方法回顾性分析2017年1月至2019年12月我科收治的63例外耳道病灶切除并皮肤缺损患者,根据修复材料分为试验组(n=32)和对照组(n=31)。试验组用脱细胞异体真皮修复外耳道皮肤缺损,对照组用后耳或大腿内侧刃厚皮片修复。比较两组手术时间、术后疼痛程度、移植物上皮化时间、术后并发症情况。结果试验组与对照组的手术时间、术后疼痛程度评分、术后并发症率比较,差异有统计学意义(P<0.05),试验组手术时间更短,术后疼痛程度更轻,并发症率更低;而两组上皮化时间比较,差异无统计学意义(P>0.05)。结论相比自体刃厚皮片,使用脱细胞异体真皮节省了手术时间,减轻了术后痛苦,避免了额外的手术瘢痕,术后并发症更少,是一种更为理想的修复外耳道皮肤缺损材料。Objective To compare the efficacies of acellular allogeneic dermis and autologous split-thickness skin in repairing skin defect of external auditory canal.Methods A total of 63 patients with external auditory canal lesion resection and skin defect admitted to our department from January 2017 to December 2019 were analyzed retrospectively.And they were divided into the experimental group(n=32)and the control group(n=31)according to the repair materials.The experimental group was treated with acellular allogeneic dermis to repair the skin defect of external auditory canal,while the control group was treated with split-thickness skin of the back ear or inner thigh to repair.The operation time,postoperative pain degree,graft epithelization time and postoperative complications were compared between the two groups.Results There were statistically significant differences in operation time,postoperative pain score and incidence of postoperative complications between the experimental group and the control group(P<0.05).In the experimental group,the operation time was shorter,the postoperative pain degree was lighter,and the incidence of complications was lower than those in the control group.However,there was no statistically significant difference in the epithelialization time between the two groups(P>0.05).Conclusion Compared with autologous split-thickness skin,acellular allogeneic dermis can save the operation time,reduce the postoperative pain,avoid the extra surgical scars and decrease the incidence of postoperative complications.Therefore,it is an ideal material for repairing skin defect of external auditory canal.
