补中益气汤加味联合生物反馈电刺激治疗围绝经期妇女盆底功能障碍性疾病的效果及其对盆底肌力的影响

摘　　要：目的补中益气汤加味联合生物反馈电刺激治疗围绝经期妇女盆底功能障碍性疾病的效果及盆底肌力的影响。方法选择临沂市中医医院妇科2019年6月至2020年6月围绝经期妇女盆底功能障碍性疾病的患者60例,随机分为两组,对照组30例予生物反馈电刺激治疗,治疗组30例予中药口服+生物反馈电刺激治疗。治疗2个月后,比较两组治疗前及治疗后临床疗效、阴道收缩肌电值、阴道静息肌电值、尿失禁、盆底脏器脱垂及血清转化生长因子-β1(TGF-β1)、基质金属蛋白酶(MMP)-3、TIMP-2水平变化情况。结果治疗2个月后治疗组总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05)。两组患者治疗后收缩肌电值、阴道静息肌电值均优于治疗前,且治疗后治疗组优于对照组,差异均有统计学意义(P<0.05)。治疗后治疗组患者的尿失禁改善总有效率为96.67%,高于对照组的76.67%,差异有统计学意义(P<0.05)。两组患者治疗后盆底脏器脱垂改善情况均优于本组治疗前,且治疗后治疗组优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后血清TGF-β1,TIMP-2水平均较本组治疗前显著升高,MMP-3水平显著降低,差异有统计学意义(P<0.05)。且治疗后,治疗组TGF-β1,TIMP-2水平高于对照组,而MMP-3水平低于对照组,差异有统计学意义(P<0.05)。结论通过补中益气汤加味联合生物反馈对围绝经期盆底功能障碍的患者有较好的临床效果,可以有效增强盆底肌力,改善尿失禁及盆底脏器脱垂,且未出现明显不良反应,疗效确切。Objective To investigate the efficacy of modified Buzhong Yiqi Decoction combined with biofeedback electrical stimulation in the treatment of perimenopausal women with pelvic floor dysfunction(PFD)and its effect on pelvic floor muscle strength.Methods A total of 60 perimenopausal women with PFD in the Department of Gynecology in Linyi Traditional Chinese Medical Hospital from June 2019 to June 2020 were randomly divided into two groups,with 30 cases in each group.30 cases in the control group were treated with biofeedback electrical stimulation,while 30 cases in the treatment group were treated with oral Chinese materia medica+biofeedback electrical stimulation.After 2 months of treatment,the clinical efficacy,value of vaginal contraction electromyography(EMG),value of vaginal resting EMG,urinary incontinence,pelvic organ prolapse and changes in serum transforming growth factor-β1(TGF-β1),matrix metalloproteinase(MMP)-3 and tissue inhibitor of metalloproteinases-2(TIMP-2)levels were compared between the two groups before and after treatment.Results After 2 m onths of treatm ent,the total effective rate of the treatm ent group was 93.33%,which was higher than 73.33%of the control group,with statistically significant difference(P<0.05).The systolic myoelectric value and vaginal resting myoelectric value of the two groups were better than before treatment,and the treatment group was better than the control group after treatment,the difference was statistically significant(P<0.05).After treatment,the total effective rate of improvement of urinary incontinence in the treatment group was 96.67%,which was higher than 76.67%in the control group,with statistically significant difference(P<0.05).The improvement of pelvic organ prolapse after treatment in the two groups was better than that before treatment,and the treatment group was better than the control group after treatment,the difference was statistically significant(P<0.05).Two groups of patients after treatment serum TGF-βThe levels of MMP-1 and TIMP-2 were sign
