摘 要:目的研究低剂量阿奇霉素治疗慢性阻塞性肺疾病对患者肺功能及血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)水平的影响。方法选取2019年1月至2020年6月随州市曾都医院内一科收治的80例慢性阻塞性肺疾病患者作为研究对象,根据随机数字表法将其分为对照组和试验组,每组各40例。对照组采用沙美特罗替卡松粉吸入剂吸入治疗,试验组在对照组基础上采用低剂量阿奇霉素分散片口服治疗,1个月为1个疗程,两组患者均连续治疗3个疗程。比较治疗前后两组运动耐力、生活质量评分、肺功能以及血清CRP、TNF-α、IL-8水平。结果与治疗前相比,治疗后两组6 min步行距离(6MWD)距离均增大,且试验组大于对照组,1 s用力呼气量(FEV1)、1 s用力呼气量/用力肺活量(FEV1/FVC)以及深吸气量(IC)水平均升高,且试验组均高于对照组,而圣乔治呼吸疾病问卷评分(SGRQ)以及血清CRP、TNF-α、IL-8水平均降低,且试验组均低于对照组(P<0.05)。结论低剂量阿奇霉素治疗慢性阻塞性肺疾病能够增强患者的肺功能,抑制炎性因子的分泌与表达,同时可提高其运动耐力,改善生活质量。Objective To investigate the impacts of low-dose azithromycin on pulmonary function and serum levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 80 patients with COPD admitted to and treated in the Department I of Internal Medicine in Hospital of Zengdu District in Suizhou City from January 2019 to June 2020 were selected as the research objects,and they were divided into the control group(n=40)and the experimental group(n=40)according to the random number table method.The control group was treated with inhalation of salmeterol and fluticasone propionate powder,while the experimental group was treated with oral administration of low-dose azithromycin dispersible tablets on the basis of the control group.One month was a course of treatment,and both groups were treated continuously for three courses of treatment.The exercise endurance,quality of life(QoL)score,pulmonary function and serum levels of CRP,TNF-αand IL-8 were compared between the two groups before and after treatment.Results Compared with that before treatment,the 6-minute walking distance(6MWD)of both groups increased after treatment,and the 6MWD of experimental group was farther than that of the control group.The levels of 1s forced expiratory volume,1 s forced expiratory volume/forced vital capacity and deep breath all increased,and those of the experimental group were all higher than those of the control group.While St.George's respiratory disease questionnaire and serum levels of CRP,TNF-α,IL-8 all decreased,those of the experimental group were lower than those of the control group(all P<0.05).Conclusion Low-dose azithromycin in the treatment of COPD can enhance the pulmonary function of patients,inhibit the secretion and expression of inflammatory factors,increase their exercise endurance and boost their QoL.
