摘 要:目的探讨化疗所致味觉改变(CITA)在乳腺癌患者中的发生情况及其对营养风险、心理痛苦和生活质量的影响。方法选取2019年5月至2020年5月于福建省肿瘤医院诊治的175例乳腺癌患者作为研究对象,采用化疗相关味觉改变量表(CiTAS)、营养风险筛查量表(NRS-2002)、心理痛苦温度计(DT)、乳腺癌生活质量测定量表(FACT-B)评估乳腺癌化疗患者的味觉改变、营养风险、心理痛苦程度和生活质量,并比较味觉改变与营养风险、心理痛苦程度和生活质量的关系。结果乳腺癌化疗患者74.9%出现味觉改变。年龄、文化程度、婚姻状况、是否手术、化疗次数、合用靶向药物治疗、雌激素受体、孕激素受体与CiTAS总分无关,差异无统计学意义(P>0.05)。CiTAS总分与生活质量得分呈负相关,与心理痛苦、营养风险得分呈正相关,差异有统计学意义(P<0.05)。含蒽环类方案组与含紫杉类方案组中味觉改变得分、生活质量、心理痛苦以及营养风险差异无统计学意义(P>0.05)。结论化疗所致味觉改变加重乳腺癌化疗患者营养风险,增加患者心理痛苦,降低患者的生活质量,需引起临床医护人员的重视,并采取必要措施进行防治。Objective To investigate the occurrence of chemotherapy induced taste alteration(CITA)in patients with breast cancer and its impacts on nutritional risk,psychological distress and quality of life(QoL).Methods A total of 175 patients with breast carcinoma diagnosed and treated in Fujian Cancer Hospital from May 2019 to May 2020 were selected as the study objects.The Chemotherapy induced Taste Alteration Scale(CiTAS),Nutritional Risk Screening Scale(NRS-2002),the Distress Thermometer(DT),and the Functional Assessment of Cancer Therapy-Breast(FACT-B)were adopted to assess taste alteration,nutritional risk,degree of psychological distress and QoL in patients with breast cancer who underwent chemotherapy.Meanwhile,the relationships between taste alteration and nutritional risk,degree of psychological distress and QoL were compared.Results 74.9%of breast cancer patients undergoing chemotherapy had taste alterations.There were no relationships between the patients'age,education level,marital status,whether they had received surgery,the number of chemotherapy,estrogen receptor,progesterone receptor and total CiTAS score,with difference was not statistically significant(P>0.05).The total CiTAS score was negatively correlated with QoL score and positively correlated with the scores of psychological distress and nutritional risk,with difference was statistically significant(P<0.05).There were no statistically significant differences betw een the group with anthracyclines-based chemotherapy and the group with paclitaxel-based chemotherapy in terms of the scores of taste alteration,QoL,psychological distress,and nutritional risk,with difference was not statistically significant(P>0.05).Conclusion CITA can aggravate the nutritional risk,increase psychological distress and reduce QoL of breast cancer patients undergoing chemotherapy.Therefore,clinical medical staff should pay attention to it and take necessary measures to prevent and treat it.
