摘 要:糖尿病肾病是糖尿病的主要并发症之一,也是导致肾衰竭的主要原因之一。低蛋白饮食[0.6 g/(kg·d)]是糖尿病肾病治疗的基础。本文通过对低蛋白饮食可以减少尿蛋白的排出、缓解糖尿病酮症酸中毒临床症状、减轻胰岛素抵抗、降低糖化血红蛋白、改善脂肪代谢、改善钙磷代谢紊乱、降低内源性尿酸生成等多个方面阐述其具有保护肾功能的作用。本文同时也阐述了临床上实施极低蛋白饮食[0.3 g/(kg·d)]时的困难及相应解决措施。另外,在实施低蛋白饮食时应注意避免营养不良的发生。同时也建议科研机构可以开发出更多的医用菜谱或医用低蛋白食品。Diabetic nephropathy is one of the major complications of diabetes mellitus and one of the main causes of renal failure.A low-protein diet[0.6 g/(kg·d)]is the basis of diabetic nephropathy treatment.In this paper,the role of low protein diet in protecting renal function by reducing urinary protein excretion,alleviating clinical symptoms of diabetic ketoacidosis,reducing insulin resistance,lowering glycated hemoglobin,improving fat metabolism,improving calcium and phosphorus metabolism disorder,and reducing endogenous uric acid production will be elaborated.This paper also describes the clinical difficulties in implementing a very low protein diet[0.3 g/(kg·d)]and the corresponding measures to solve them.In addition,care should be taken to avoid malnutrition when implementing a low-protein diet.It is also suggested that scientific institutions develop more medical rec ipes or medical low-protein food.
