PPH术联合套扎术治疗女性直肠前突合并直肠黏膜内脱垂的便秘效果分析

摘　　要：目的探讨直肠黏膜环切术(PPH)联合套扎术治疗女性直肠前突合并直肠黏膜内脱垂导致的出口梗阻性便秘的临床效果。方法选取2017年10月至2020年6月90例女性直肠前突合并直肠黏膜内脱垂致出口梗阻性便秘患者随机分为两组,联合组行PPH联合套扎术,PPH组行PPH。采用SPSS26.0软件进行分析,围术期各项指标及wexner便秘评分以(x±s)描述,采用独立t检验;术后并发症发生情况等计数资料采用χ^(2)检验;临床疗效采用秩和检验。P<0.05差异有统计学意义。结果两组围术期各项指标比较差异无统计学意义(P>0.05)。术前两组患者wexner评分差异无统计学意义(P>0.05),术后1、3、6个月联合组患者的wexner评分明显低于PPH组(P<0.05);联合组并发症发生率为13.3%低于PPH组并发症发生率31.1%(P<0.05);联合组总有效率为97.8%高于PPH组总有效率为86.7%,以上差异均有统计学意义(P<0.05)。结论PPH联合套扎术治疗女性直肠前突合并直肠黏膜内脱垂型便秘效果明显,术后复发率较低,可显著改善患者排便情况。Objective To analyze the clinical effect of procedure for prolapse and hemorrhoids(PPH)combined with ligation in the treatment of female outlet obstruction constipation caused by rectocele combined with rectum mucosal prolapse.Methods From October 2017 to June 2020,90 female patients of outlet obstruction constipation caused by rectocele combined with rectum mucosal prolapse were randomly divided into two groups,The unite group received PPH combined ligation.While the PPH group received PPH.SPSS26.0 software was used for analysis.Perioperative indicators and Wexner constipation score were described as (x±s),and independent t test was used.The occurrence of postoperative complications were tested by chi square test.The rank sum test was used for the clinical efficacy.P<0.05 was considered as statistically significant difference.Results There was no significant difference in perioperative indicators between the two groups(P>0.05).There was no significant difference in wexner score before surgery between the two groups(P>0.05),and the wexner score of the unite group was significantly lower than the PPH group in postoperative 1,3 and 6 months(P<0.05).The complication rate of the combined group was 13.3%,which was lower than that in the PPH group(31.1%,P<0.05).The total effective rate of the combined group was 97.8%,which was higher than that of PPH group(86.7),with significantly difference(P<0.05).Conclusions PPH combined with ligation in the treatment of female outlet obstruction constipation caused by rectocele combined with rectum mucosal prolapse has obvious effect,and the postoperative recurrence rate is low,which can significantly improve the patient’s defecation situation.
