摘 要:目的基于16S rRNA高通量测序技术,分析青蒿琥酯片对艾滋病患者肠道细菌群落结构的影响。方法纳入40名健康受试者和ART后免疫重建不良HIV/AIDS患者62例,包括ART组20例和ART联合青蒿琥酯片组(简称青蒿琥酯片组)42例,共治疗24周,利用16S rRNA高通量测序技术观察上述两种干预方法对患者肠道菌群结构的影响。结果基线时,ART组和青蒿琥酯片组患者血浆CD4细胞计数未见明显差异,经24周治疗后,两组患者血浆CD4细胞计数均升高,且青蒿琥酯片组患者[(237.99±71.61)个/μL]明显高于ART组[(198.34±53.42)个/μL],差异有统计学意义(t=-2.534,P=0.013)。主成分分析结果表明,ART组和青蒿琥酯片组患者基线粪便肠道微生物组成基线相似,而该两组患者与健康受试者之间微生物的组成差异较大。经24周治疗后,患者粪便微生物菌落结构及组成分析发现,不同治疗组患者粪便菌群结构发生明显改变;其中青蒿琥酯片组治疗后,与健康受试者较为接近,而ART后与健康受试者差异显著。结论HIV/AIDS患者肠道微生物群发生明显改变,青蒿琥酯片干预肠道失调和抑制免疫激活可能是改善HIV-1感染个体免疫重建的新策略。Objective To analyze the effects of artesunate tablets on the intestinal bacterial community of people living with HIV(PLWH)based on 16S rRNA high-throughput sequencing technology.Methods A total of 20 healthy controls and 62 PLWH with poor immune reconstitution after ART were enrolled.Among 62 PLWH,20 received the ART alone and 42 received both ART and 24-week treatment of artesunate tablets.The high-throughput sequencing technology was used to observe the effects of the two interventions on the intestinal microflora environments.Results The number of plasma CD4+cells increased in the two groups after 24 weeks of treatment.The CD4+cells in the artesunate group[(237.99±71.61)//uL]were higher than that in the ART alone group[(198.34±53.42)/uL](P<0.05).The results of principal component analysis(PCA)showed that the fecal and intestinal microbial composition in two PLWH groups before treatment was similar.However,the microbial compositions in the two PLWH groups were significantly different from those in healthy individuals.After 24 weeks of treatment,the fecal microbes colonies and their compositions changed significantly.They were similar between the healthy controls and patients who received the artesunate tablets and ART,but significantly different between healthy controls and patients who received ART only.Conclusion Intestinal microbiota changed significantly among PLWH after treatments.Interfering intestinal dysregulation and inhibiting immune activation may be a new strategy to improve the immune reconstruction of PLWH.
