摘　　要：目的分析山东省2014-2018年MSM哨点HIV-1新发感染率,全面了解新发感染情况。方法按照全国艾滋病哨点监测实施方案要求,用实验室方法对2014-2018年采集的MSM哨点人群血液样本进行HIV抗体检测和HIV-1新发感染检测,计算HIV抗体阳性率和HIV-1新发感染率等指标。结果2014-2018年山东省8个MSM哨点共监测15820人,HIV抗体阳性656人,总阳性率为4.15%。2014-2018年HIV抗体阳性率分别为4.41%(135/3060)、4.55%(146/3210)、4.57%(151/3307)、3.91%(124/3169)和3.25%(100/3074);HIV-1新发感染率分别为4.16%、3.25%、3.44%、2.77%和3.06%。2014-2018年山东省8个MSM哨点中,HIV抗体阳性率和HIV-1新发感染率排前三位的是烟台、济南和淄博市,抗体阳性率分别是8.84%(179/2024)、6.92%(148/2140)和6.33%(119/1879);HIV-1新发感染率分别为7.42%、6.29%和6.66%,远高于全省总体水平,其他哨点HIV抗体阳性率均低于全省总体水平。结论山东省MSM的HIV感染情况相对严重,不同哨点之间差别明显应继续加强宣传干预等措施。Objective:To calculate the annual HIV-1 incidence among Men Who Have Sex with Men(MSM)followed up in sentinel sites of Shandong Province from 2014-2018,aiming to estimate the overall HIV incident among MSM in the whole province.Methods According to the sentinel surveillance protocol,we diagnosed the HIV infection based on the identification of HIV antibodies through laboratory tests to estimate the recent HIV infection among MSM followed up in the sentinel sites in Shandong Province.Results A total of 15,820 individuals were recruited from 8 MSM sentinel sites during 2014-2018,and 656 individuals were HIV antibody positive.The overall HIV positivity rate was 4.15%(656/15820).From 2014 to 2018,the annual HIV antibody positivity rate was 4.41%(135/3060),4.55%(146/3210),4.57%(151/3307),3.91%(124/3169),and 3.25%(100/3074),respectively.The estimated HIV-1 incidence was 4.16%in 2014,3.25%in 2015,3.44%in 2016,2.77%in 2017,and 3.06%in 2018.In the 8 MSM sentinel sites,Yantai,Jinan,and Zibo ranked in the top three in terms of the HIV antibody positivity rate and HIV-1 incidence.The HIV antibody positivity rate was 8.84%(179/2024)in Yantai,6.92%(148/2140)in Jinan,and 6.33%(119/1879)in Zibo.The estimated HIV-1 incidence was 7.42%in Yantai,6.29%in Jinan,and 6.66%in Zibo,much higher than the provincial average.The figure from other sites was lower than the provincial average.Conclusions The HIV epidemic situation among MSM in Shandong province was serious.There were significant differences in HIV antibody positivity rates and HIV incidence cross sites.Further effective HIV prevention strategies are needed for MSM in Shandong province.
