摘  要:目的:探讨剪切波弹性成像技术及TI-RADS在诊断甲状腺结节中的应用。方法:选择2019-12~2020-06我院收入甲状腺结节患者80例共125个结节,术前均对甲状腺结节行剪切波弹性成像技术测量剪切波速度值(SWV),同时按照TI-RADS分类标准进行分级诊断,以术后病理结果为金标准。结果:125个结节中,良性的有82个,包括66个结节性甲状腺肿和16个甲状腺腺瘤;恶性的有43个,包括36个甲状腺乳头状癌、2个甲状腺髓样癌、4个甲状腺滤泡状癌、1个甲状腺淋巴瘤。剪切波弹性成像技术联合TI-RADS对甲状腺结节良恶性诊断效能明显大于单一方法,具有统计学意义(P<0.05)。结论:弹性成像技术及TI-RADS分级明显提高甲状腺结节良恶性的诊断率。Objective:To explore the application of shear wave elastography and TI-RADS in the diagnosis of thyroid nodule.Methods:A total of 125 thyroid nodules were selected from 80 patients admitted to our hospital from December 2019 to June 2020.All thyroid nodules were measured by shear wave velocity(SWV)using shear wave elastography before surgery.Meanwhile,graded diagnosis was made according to TI-RADS classification standard,and postoperative pathological results were taken as the gold standard.Results:Of the 125 nodules,82 were benign,including 66 nodular goiters and 16 thyroid adenomas.There were 43 malignant cases,including 36 papillary thyroid carcinoma,2 medullary thyroid carcinoma,4 follicular thyroid carcinoma,and 1 thyroid lymphoma.The efficacy of shear wave elastography combined with TI-RADS in the diagnosis of benign and malignant thyroid nodules was significantly higher than that of the single method,and the difference was statistically significant(P<0.05).Conclusion:Sear-wave elastography combined with TI-RADS grading detection can significantly improve the diagnosis rate of benign and malignant thyroid nodules.
