针刺百会八阵穴对缺血性脑卒中运动性失语患者语言功能及脑血流的影响

摘　　要：目的:观察针刺百会八阵穴治疗缺血性脑卒中运动性失语患者的临床疗效及对脑血流的影响。方法:将符合纳入标准的80例患者随机分为观察组和对照组各40例。两组均接受许尔失语症刺激疗法进行语言功能训练,对照组接受头针针刺治疗,观察组接受百会八阵穴针刺治疗,治疗8周。使用中国康复研究中心汉语标准失语症检查量表观察治疗前后患者语言功能评分的变化,并评价临床疗效;观察患者治疗前后左侧大脑前动脉(ACA)、中动脉(MCA)、后动脉(PCA)的平均血流速度变化。结果:治疗后观察组与对照组语言功能各项评分、总分及左侧大脑ACA、MCA平均血流速度均较治疗前升高(P<0.05),且观察组高于对照组(P<0.05)。治疗后两组左侧大脑PCA平均血流速度与治疗前比较差异无统计学意义(P>0.05),观察组与对照组比较差异亦无统计学意义(P>0.05)。观察组与对照组临床疗效总有效率分别为92.31%、76.32%,差异有统计学意义(P<0.05)。结论:针刺百会八阵穴可以改善缺血性脑卒中运动性失语患者的语言功能,同时可以改善左侧大脑ACA、MCA平均血流速度,临床疗效显著。Objective:To observe the clinical effect of acupuncture at Baihui Bazhen acupoints on patients with motor aphasia after ischemic stroke and its effect on cerebral blood flow.Methods:A total of 80 patients who met the inclusion criteria were randomly divided into the observation group and the control group,with 40 cases in each group.Both groups received Schuell stimulation for language function training,the control group received scalp acupuncture,and the observation group received acupuncture at Baihui Bazhen acupoints.The treatment was for 8 weeks.Chinese Standard Aphasia Scale of China Rehabilitation Research Center was used to observe the changes in the patient’s language function scores before and after treatment,and evaluate the clinical effect.The changes of the average blood flow velocity of left anterior cerebral artery(ACA),middle cerebral artery(MCA)and posterior cerebral artery(PCA)before and after treatment were observed.Results:After treatment,the scores of language function each sub item,total score and average blood flow velocity of left cerebral ACA and MCA of the observation and control group were higher than those before treatment(P<0.05),and the observation group was higher than the control group(P<0.05).There was no statistical difference in average blood flow velocity of left cerebral PCA before and after treatment aoocrding to two groups(P>0.05),and there was no statistical difference between the observation and control group(P>0.05).The total effective rate of the observation group and the control group was 92.31%and 76.32%,respectively,which was with statistical differences(P<0.05).Conclusion:Acupuncture at Baihui Bazhen acupoints can significantly improve the language function of the patients with aphasia after ischemic stroke,and it can improve the average blood flow velocity of ACA and MCA in the left brain,of which the clinical effect is significant.
