通窍活血汤与穴位敷贴辅助西药治疗血管性痴呆患者的效果及对血清神经递质的影响  

Effect of Tongqiaoxue Huoxue Decoction and Acupoint Application in the Treatment of Vascular Dementia Patients and the Effect on Serum Neurotransmitters

摘　　要：目的探讨通窍活血汤与穴位敷贴辅助西药治疗血管性痴呆(VD)患者的效果及对血清神经递质的影响。方法选取2017年5月至2019年6月新郑中医院VD患者92例,依据随机数字表法分组,各46例。常规组接受西药治疗,联合组在常规组基础上接受通窍活血汤与穴位敷贴治疗。比较两组疗效、治疗前后血清神经递质[乙酰胆碱(ACh)、5-羟色胺(5-HT)、去甲肾上腺素(NE)、多巴胺(DA)]水平,采用简易智能精神状态检查量表(MMSE)评估患者精神状况,采用日常生活能力量表(ADL)评估患者日常生活能力。结果联合组治疗总有效率(91.30%)高于常规组(69.57%)(P<0.05);治疗后,联合组血清ACh、5-HT、NE、DA水平高于常规组(P<0.05);联合组治疗后MMSE评分高于常规组,ADL评分低于常规组(P<0.05)。结论通窍活血汤与穴位敷贴辅助西药治疗VD患者疗效显著,可上调神经递质水平,明显改善智力状态,提高日常生活能力。Objective To investigate the effect of Tongqiaoxue Huoxue Decoction and acupoint application assisted by western medicine in the treatment of vascular dementia(VD)and the effect on serum neurotransmitters.Methods A total of 92 patients with VD in Xinzheng Hospital of Traditional Chinese Medicine from May 2017 to June 2019 were selected and divided into groups according to random number table method,46 patients in each group.The conventional group was treated with western medicine,and the combined group was treated with Tongqiaoxue Huoxue Decoction and acupoint application on the basis of the conventional group.The therapeutic effect,serum neurotransmitters[acetylcholine(Ach),5-HT(5-HT),norepinephrine(NE),dopamine(DA)]levels were compared between the two groups.Patient mental condition was assessed using the mini-mental state examination(MMSE).The activity of daily living(ADL)was used to assess the patient’s daily ability.Results The total effective rate of combined group(91.30%)was higher than that of conventional group(69.57%)(P<0.05).After treatment,the serum levels of Ach,5-HT,NE and DA in combined group were higher than those in normal group(P<0.05).After treatment,the MMSE score of combined group was higher than that of conventional group,while the ADL score of combined group was lower than that of conventional group(P<0.05).Conclusion Tongqiao Huoxue Decoction and acupoint application in the treatment of patients with VD have a significant effect,can increase the level of neurotransmitters,improve the mental state significantly,and improve the ability of daily life.
