股前外侧Flow-through皮瓣修复手部创面伴动脉缺损的效果

摘　　要：目的探讨股前外侧Flow-through皮瓣修复手部创面伴动脉缺损的效果。方法选取手部创面伴动脉缺损病人70例,随机分成两组,各35例。观察组行股前外侧Flow-through皮瓣修复创面,对照组行游离腹壁下动脉穿支皮瓣修复。观察两组病人疗效及不良反应情况,检测两组病人手术前后血标本红细胞沉积率(ESR)、超敏C反应蛋白(hs-CRP)及降钙素原(PCT)含量。结果观察组、对照组总有效率分别为94.29%、80.00%,差异有统计学意义(χ^(2)=7.620,P<0.05)。两组病人手术后血标本ESR、hs-CRP和PCT含量较手术前升高,且观察组治疗前后差值低于对照组,差异有统计学意义(t=5.299~22.750,P<0.01)。观察组不良反应发生率为11.43%,对照组为22.86%,两组比较差异有显著性(χ^(2)=4.188,P<0.01)。结论股前外侧Flow-through皮瓣修复手部创面伴动脉缺损效果好,且机体炎症反应程度较轻,更利于病人康复。Objective To investigate the effect of anterolateral thigh flow-through flap in repairing hand wounds with arterial defects.Methods A total of 70 patients with hand wounds and arterial defects were enrolled and randomly divided into observation group and control group,with 35 patients in each group.The patients in the observation group underwent wound repairing with anterolateral thigh flow-through flap,and those in the control group underwent wound repairing with free perforator flap of the inferior abdominal artery.Clinical outcome and adverse reactions were observed for both groups,and serum erythrocyte sedimentation rate(ESR),hypersensitive C-reactive protein(hs-CRP),and procalcitonin(PCT)were measured before and after surgery.Results There was a significant difference in overall response rate between the observation group and the control group(94.29%vs 80.00%,χ^(2)=7.620,P<0.05).Both groups had significant changes in serum ESR,hs-CRP,and PCT after surgery,and the observation group had significantly greater changes than the control group(t=5.299-22.750,P<0.01).Conclusion Anterolateral thigh flow-through flap has a good effect in repairing hand wounds with arterial defects,with slight inflammatory response,and thus helps with patient rehabilitation.
