预防性使用抗生素对腹股沟疝手术切口愈合的影响

摘　　要：目的探究腹股沟疝手术前预防性使用抗生素对手术切口愈合的影响。方法以2018年9月—2020年4月来该院诊治的163例腹股沟疝患者为研究对象,以随机数表法分为两组,两组患者均常规行疝环填充式无张力修补术,对照组术前未预防性使用抗生素,观察组术前预防性使用抗生素注射用五水头孢唑林,对比两组手术切口感染率、手术前后体温、外周血象白细胞水平变化情况。结果对照组术后共8例患者切口感染,切口感染发生率为7.01%,观察组术后共有1例患者手术切口感染,感染率为2.04%,差异无统计学意义(χ^(2)=0.813,P>0.05);观察组术后1 d、术后2 d、术后3 d时体温分别为(37.91±0.09)、(37.39±0.26)、(37.09±0.12)℃,与对照组比较,差异无统计学意义(t=0.028、0.007、0.063,P>0.05);观察组术后外周血象白细胞水平为(6.15±0.85)×109/L,与对照组比较,差异无统计学意义(t=0.258,P>0.05)。结论腹股沟疝手术前没有必要预防性使用抗生素。Objective To explore the effect of preventive use of antibiotics on the healing of surgical incisions before inguinal hernia surgery.Methods A total of 163 inguinal hernia patients who came to the hospital for diagnosis and treatment from September 2018 to April 2020 were taken as the research objects.They were divided into two groups by a random number table.The two groups of patients underwent routine tension-free repair of hernia ring filling.The control group did not use antibiotics prophylactically before operation,and the observation group used cefazolin pentahydrate for antibiotic injections before operation.The infection rate of surgical incisions,body temperature before and after operation,and changes in peripheral blood white blood cell levels were compared between the two groups.Results A total of 8 patients in the control group had wound infection,the incidence of wound infection was 7.01%,and there was a total of 1 patient in the observation group who had surgical wound infection after surgery,and the infection rate was 2.04%,and the difference was not statistically significant(χ^(2)=0.813,P>0.05);the body temperature of the observation group at 1 d,2 d and 3 d after operation was(37.91±0.09)℃,(37.39±0.26)℃,(37.09±0.12)℃,compared with the control group,there not statistically significant difference(t=0.028,0.007,0.063,P>0.05);the white blood cell level of the observation group after operation was(6.15±0.85)×109/L,compared with the control group,there was no statistically significant difference(t=0.258,P>0.05).Conclusion It is not necessary to use antibiotics prophylactically before inguinal hernia surgery.
