摘　　要：目的研究支气管内超声引导针吸活检术(EBUS-TBNA)检查和血清miR-20a检测对肺癌的诊断价值。方法选取2019年1月至2020年7月于该院确诊为肺癌的60患者纳入研究作为肺癌组;另外,选取同期于该院治疗的肺部良性病变且无肿瘤史的患者60例作为良性病变组。分别对两组患者进行EBUS-TBNA检查和血清miR-20a检测;比较两组患者的EBUS-TBNA检查以及miR-20a检测结果;比较不同病理分型及术前分期肺癌患者的EBUS-TBNA检查以及miR-20a检测结果;对两种方法单独及联合使用对肺癌诊断的效能进行分析。结果肺癌组EBUS-TBNA检查的阳性率显著高于良性病变组(P<0.05);不同病理分型及术前分期肺癌患者的EBUS-TBNA检查的阳性率比较差异均有统计学意义(P<0.05);肺癌组患者的血清miR-20a相对表达水平显著高于良性病变组(P<0.05);不同病理分型及术前分期肺癌患者的血清miR-20a相对表达水平比较差异有统计学意义(P<0.05)。EBUS-TBNA检查联合血清miR-20a检测对肺癌诊断的特异度高于两种方法单独使用。结论EBUS-TBNA检查和血清miR-20a检测有助于肺癌的诊断。Objective To study the diagnostic value of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA)and serum miR-20a in lung cancer.Methods A total of 60 patients with lung cancer diagnosed in the hospital from January 2019 to July 2020 were enrolled as the lung cancer group,meanwhile 60 patients with benign lung lesions and no tumor history were enrolled as the benign lesion group.EBUS-TBNA and serum miR-20a detection were performed respectively in the two groups.The results of EBUS-TBNA and miR-20a detection were compared between the two groups,The results of EBUS-TBNA and miR-20a detection were comapared among lung cancer patients with different pathological types and preoperative stages.The effectiveness of the two methods alone and in combination were analyze in the diagnosis of lung cancer.Results The positive rate of EBUS-TBNA in patients with lung cancer were significantly higher than that in patients with benign lesions(P<0.05);the positive rates of EBUS-TBNA in patients with different pathological types and preoperative stages of lung cancer were significantly different(P<0.05).The relative expression level of serum miR-20a in the lung cancer group was significantly higher than that in the benign lesion group(P<0.05);the relative expression level of serum miR-20a in patients with different pathological types and preoperative stages of lung cancer was significantly different(P<0.05).The specificity of EBUS-TBNA combined with serum miR-20a detection in the diagnosis of lung cancer is higher than those of the two methods alone.Conclusion EBUS-TBNA and serum miR-20a detection are helpful for the diagnosis of lung cancer.
