摘 要:目的:探讨微动力负压护创对糖尿病大鼠创面血管生成及其受体表达的影响。方法:对50只无特定病原体(SPF)级斯泼累格·多雷(SD)大鼠(8周龄,体质量200~250 g)进行4周高糖高脂喂养,构建糖尿病创面大鼠模型,使用随机数字表法将大鼠分为对照组与观察组,每组25只。对照组2 d进行一次常规换药,观察组1周更换一次敷料,两组连续治疗14 d。比较两组大鼠的血糖、创面血流量与创面血管密度;比较两组创面血管内皮生长因子(VEGF)、血管内皮生长因子受体1(Fit-1)、血管生成素(Ang)-1和Ang-2受体、酪氨酸激酶受体(Tie-2)mRNA的表达水平。结果:观察组大鼠治疗当天、治疗后1周、治疗后2周创面血流量显著高于对照组(P<0.05);治疗后1周、治疗后2周血管密度显著高于对照组(P<0.05);两组治疗期间各时间段血糖水平比较,差异无统计学意义(P>0.05);治疗前两组大鼠VEGF,Fit-1,Ang-1,Ang-2,Tie-2的mRNA相对表达量比较,差异无统计学意义(P>0.05);治疗当天、治疗后1周观察组大鼠VEGF,Fit-1,Ang-1,Ang-2的mRNA相对表达量均明显高于对照组(P<0.05);治疗后2周观察组大鼠VEGF,Ang-1的mRNA与对照组比较,差异无统计学意义(P>0.05);治疗后1周及治疗后2周,对照组Tie-2的mRNA相对表达量显著高于观察组(P<0.05);治疗后2周对照组Ang-2mRNA显著高于观察组(P<0.05)。结论:微动力负压护创技术能通过改变糖尿病溃疡大鼠创面细胞因子水平,促进创面新生血管形成,进而促进创面愈合。Objective:To investigate the effect of microdynamic negative pressure wound care on the angiogenesis and receptor expression of the wound in diabetic rats.Methods:Fifty specific pathogen free(SPF)-free speleger doray(SD)rats(8 weeks old,weight 200~250 g)were fed high-sugar and high-fat for 4 weeks to construct a diabetic wound rat model using a random number table method,the rats were divided into control group and observation group,each with 25 rats.The control group received a regular dressing change every 2 days,and the observation group changed dressings once a week.The two groups were treated for 14 consecutive days.The blood glucose,wound blood flow and wound blood vessel density of the two groups of rats were compared,and the wound vascular endothelial growth factor(VEGF),vascular endothelial growth factor receptor 1(Fit-1),and angiopoietin(Ang-1,Ang-2)The expression level of receptor,tyrosine kinase receptor(Tie-2)mRNA.Results:On the day of treatment,1 week after treatment,and 2 weeks after treatment,the blood flow of the wound in the observation group was significantly higher than that of the control group(P<0.05),and the blood vessel density at 1 week and 2 weeks after treatment was significantly higher than that of the control group(P<0.05),there was no significant difference in blood glucose levels between the two groups during the treatment period(P>0.05).Before treatment,the two groups of rats had vascular endothelial growth factor(VEGF),Fit-1,Ang-1,Ang-2,there was no significant difference in the relative expression of Tie-2 mRNA(PP>0.05).The relative expression of VEGF,Fit-1,Ang-1 and Ang-2 RNA in the observation group were significantly higher than those in the control group on the day of treatment and 1 week after treatment(P<0.05),there was no significant difference between VEGF and Ang-1 in the observation group and the control group 2 weeks after treatment(P>0.05).1 week after treatment and 2 weeks after treatment,the relative expression of Tie-2 in the control group The amount was significan
