抗生素联合自拟中药灌肠汤对腹腔镜治疗盆腔脓肿预后临床疗效观察

摘　　要：目的观察术前使用抗生素联合自拟中药灌肠汤对腹腔镜手术治疗盆腔脓肿预后的疗效。方法将我院100例盆腔脓肿患者随机分为对照组(46例)与观察组(54例)。对照组腹腔镜术前使用抗生素治疗盆腔脓肿,观察组在对照组治疗基础上联合自拟中药灌肠汤灌肠治疗。比较两组患者的临床疗效和术中、术后以及随访期内各项指标的差异。结果观察组临床疗效高于对照组(P<0.05);观察组术中出血量、手术时间均低于对照组(P<0.05);观察组体温、白细胞、C-反应蛋白(CRP)的恢复时间和平均住院时间均低于对照组(P<0.05);观察组随访期内痛经发生率、盆腔炎症复发率均低于对照组(P<0.05)。结论术前使用抗生素联合自拟中药灌肠汤可以改善腹腔镜手术治疗盆腔脓肿的术中情况,促进术后恢复。Objective To observe the effect of preoperative antibiotics combined with self-made TCM enema decoction on prognosis of laparoscopic treatment of pelvic abscess.Methods A total of 100 patients with pelvic abscess were randomly divided into control group(n=46) and observation group(n=54).Control group was treated with antibiotics pbservation group was treated with self-made TCM enema decoction on the basis of control group.The clinical efficacy and the differences of various indexes during operation,after operation and during the follow-up period were compared between the two groups.Results The clinical curative effect of observation group was higher than that of control group(P<0.05);after treatment,the therapeutic effects of both groups were improved,and the improvement of observation group was more obvious than that of control group(P<0.05),the amount of intraoperative blood loss and operation time in observation group were less than those in control group,and the recovery time and average hospitalization time of body temperature,white blood cells and C-reactiveprotein(CRP) in observation group were also lower than those in control group(P<0.05);the incidence of dysmenorrhea and the recurrence rate of pelvic inflammation in observation group were also lower than those in control group(P<0.05).Conclusion Preoperative use of antibiotics combined with self-made TCM enema decoction can greatly improve the intraoperative situation of laparoscopic surgery in the treatment of pelvic abscess and promote postoperative recovery.
