麝香通心滴丸对择期PCI术冠心病患者心肌和血管内皮保护作用

摘　　要：目的观察麝香通心滴丸对择期PCI术冠心病患者的心肌和血管内皮保护作用及机制。方法将我院行择期PCI术的80例冠心病患者随机分为治疗组与对照组各40例。对照组给予常规治疗,治疗组在对照组治疗基础上,术前72h给予麝香通心滴丸70mg/次,每日3次口服。评估两组的临床疗效,检测两组患者术前、术后24h、48h超敏C反应蛋白(hs-CRP)、超敏肌钙蛋白(hs-TnT)和血管性血友病因子(vWF)、内皮素-1(ET-1)表达水平。检测患者术后24h外周血CD_(4)凋亡比例和CD_(4)^(+)Foxp3^(+)水平对PCI术后患者24h hsCRP、hs-TnT和vWF、ET-1水平进行相关性分析。结果治疗组临床治疗总有效率明显高于对照组(P<0.05);术后,两组患者hs-TnT和hs-CRP水平明显高于术前(P<0.05),治疗组hs-TnT和hs-CRP水平较对照组有显著下降(P<0.05);术后24hvWF、ET-1水平明显高于术前治疗组vWF、ET-1水平较对照组明显降低(P<0.05);术后24h治疗组CD_(4)凋亡比例、CD_(4)^(+)Foxp3^(+)水平高于对照组(P<0.05);术后24hhs-TnT与hs-CRP呈正相关;两组患者vWF、ET-1水平与hs-TnT或hs-CRP水平无相关性(P>0.05)。结论择期PCI术的冠心病患者术前短期口服麝香通心滴丸能诱导机体产生具有抑炎作用CD_(4)^(+)Foxp3^(+),能降低ET-1水平,减少PCI术引起的心肌损伤。Objective To observe the myocardial and vascular endothelial protective effects and mechanisms of Shexiang Tongxin Dripping Pills on coronary heart disease patients undergoing elective PCI.Methods 80 patients with coronary heart disease who underwent elective PCI were randomly divided into treatment group and control group,40 cases each.Control group was given routine therapy,while treatment group was given Shexiang Tongxin Dripping Pill 70 mg/dose orally 3 times a day 72 h before the operation on the basis of control group.The clinical effects of the two groups were compared;the expression levels of high sensitive C-reactive protein(hs-CRP),high sensitive troponin(hs-TnT),vascular hemophilia factor(vWF) and endothelin-1(ET-1) were measured before,24 h after surgery and 48 h after surgery in both groups;the levels of CD_(4) apoptosis proportion and CD_(4)^(+)Foxp3^(+)in the peripheral blood of patients at 24 h after PCI were measured;the levels of hs-CRP,hs-TnT and vWF,and ET-1 in patients at 24 h after PCI were correlated,and the clinical effects of the two groups were compared.Results The total effective rate in treatment group was significantly higher than that of control group(P<0.05);the levels of hs-TnT and hs-CRP were significantly higher than those before PCI in both groups(P<0.05),and were significantly lower in observation group than those in control group(P<0.05);4 h vWF and ET-1 levels after PCI were significantly higher than those before PCI,and the levels of vWF and ET-1 in treatment group were significantly lower than control group(P<0.05);the CD_(4) apoptosis proportion and CD_(4)^(+)Foxp3^(+)levels in treatment group at 24 h after surgery were significantly higher than control group(P<0.05);hs-TnT and hs-CRP were positively correlated at 24 h after PCI,vWF and ET-1 levels were not correlated with hs-TnT or hsCRP levels in both groups(P<0.05).Conclusion Short-term preoperative oral administration of Shexiang Tongxin Dripping Pills in coronary heart disease patients undergoing elective PCI can indu
