放散式体外冲击波穴位治疗联合独活寄生汤对老年膝骨性关节炎疗效的影响

摘　　要：目的观察放散式体外冲击波穴位治疗联合独活寄生汤对老年膝骨性关节炎(KOA)患者疗效研究影响。方法将我院90例老年KOA患者随机分为观察组与对照组各45例,对照组给予放散式体外冲击波穴位治疗,观察组在对照组治疗基础上给予独活寄生汤,比较两组临床疗效、临床症状、骨关节炎指数(WOMAC量表评分)、膝关节功能(Lysholm-Ⅱ量表评分)、疼痛评分(VAS评分)、炎性因子[白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、氧化应激指标[超氧化物歧化酶(SOD)、一氧化氮(NO)]水平及不良反应情况。结果观察组临床总有效率高于对照组(P<0.05);治疗后,观察组腰背冷痛、乏力、关节僵硬、关节冷痛、活动受限症状积分低于对照组(P<0.05);观察组骨关节炎指数(WOMAC)量表评分、视觉模拟评分量表(VAS)评分低于对照组,膝关节功能评分(Lysholm-Ⅱ)量表评分高于对照组(P<0.05);观察组血清IL-6、IL-1、TNF-α、NO水平低于对照组,SOD水平高于对照组(P<0.05);两组不良反应发生率比较无统计学意义(P>0.05)。结论放散式体外冲击波穴位治疗联合独活寄生汤可有效改善老年KOA患者临床症状,提高患者膝关节功能,减轻疼痛,降低炎性反应与氧化应激损伤,疗效显著且安全性高。Objective To observe the effect of radial extracorporeal shock wave acupoint therapy combined with Duhuo Jisheng Decoction on lower limb function in elderly patients with knee osteoarthritis(KOA).Methods A total of 90 elderly KOA patients in were randomly divided into observation group and control group,45 cases each.Control group was given radial extracorporeal shock wave acupoint therapy,while observation group was additionally given Duhuo Jisheng Decoction.The clinical curative effect,clinical symptoms,osteoarthritis index(WOMAC),knee function(Lysholm-Ⅱ),Visual analogue scale(VAS) score,inflammatory factors [interleukin-1(IL-1),IL-6,tumor necrosis factor-α(TNF-α)],oxidative stress indexes[superoxide dismutase(SOD),nitric oxide(NO)],and adverse reactions were compared between the two groups.Results The response rate of clinical treatment in observation group was higher than that in control group P<0.05);after treatment,scores of clinical symptoms(back pain,fatigue joint stiffness joint painand activity limitation) in observation group were lower than those in control group(P<0.05);after 2 and 4 weeks of treatment,WOMAC and VAS scores in observation group were lower than those in control group,while Lysholm-Ⅱ score was higher than that in control group(P<0.05);levels of serum IL-6,IL-1,TNF-α and NO in observation group were lower than those in control group,while level of SOD was higher than that in control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Radial extracorporeal shock wave acupoint therapy combined with Duhuo Jisheng Decoction can effectively improve clinical symptoms and knee function of elderly KOA patients, alleviate pain,reduce inflammatory reactions and oxidative stress damage,with significant curative effect and high safety.
