摘　　要：目的:探讨中药封包治疗联合腹腔注射抗生素治疗原发性肝癌(PLC)合并自发性腹膜炎(SBP)的临床疗效。方法:选取2017年2月至2019年10月收治的PLC合并SBP患者50例为研究对象,随机分为研究组和对照组。对照组患者给予腹腔注射抗生素治疗,研究组患者在对照组的基础上给予中药封包治疗,疗程均为2周。观察两组患者治疗后腹部症状、体征及发热的情况,检测治疗前后炎症指标:腹水炎症因子如白细胞介素(IL)-1β、IL-17、IL-22水平,血清,白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)变化以及临床疗效。结果:研究组患者发热、腹部症状及体征的缓解时间,炎症因子的水平以及临床改善疗效均较对照组更好,差异有统计学意义(P<0.05)。结论:中药封包治疗联合腹腔注射抗生素可明显缓解PLC合并SBP患者的临床症状、体征,改善腹水及血液的炎症指标,提高有效率,具有较好的临床疗效。Objective:To explore the clinical efficacy of traditional Chinese medicine package combined with intraperitoneal injection of antibiotics in the treatment of primary liver cancer with spontaneous peritonitis(SBP).Methods:Fifty patients with primary hepatic carcinoma and SBP from February 2017 to October 2019 in our department were randomly divided into treatment group and control group.The control group was treated with intraperitoneal injection of antibiotics,while the treatment group was given traditional Chinese medicine package treatment on the basis of the control group.The course of treatment was 2 weeks.abdominal symptoms,signs and fever of change were observed before and after treatment,inflammation indexes[ascites inflammatory factor indicators interleukin-1β(IL-1β),interleukin-17(IL-17),interleukin-22(IL-22)levels,serum interleukin-6(IL-6),white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT)]were recorded before and after treatment.Results:The time to relief of fever,abdominal symptoms and signs,the level of inflammatory factors,and the efficacy of patients in the treatment group were better than those in the control group,and the difference was statistically significant.Conclusion:Chinese medicine package combined with intraperitoneal injection of antibiotics can significantly alleviate the clinical symptoms and signs of SBP,and reduce the levels of inflammatory indicators in ascites and blood,with high clinical treatment efficacy.
