痔上黏膜环切术联合外痔切剥术治疗环状混合痔患者的效果

摘　　要：目的:观察痔上黏膜环切术(PPH)联合外痔切剥术治疗环状混合痔患者的效果。方法:选取72例环状混合痔患者作为研究对象,根据系统抽样法分为对照组和研究组各36例。对照组实施外痔切剥术治疗,研究组实施PPH联合外痔切剥术,比较两组手术相关指标水平、术后疼痛程度评分、临床疗效和术后并发症发生率。结果:两组手术用时比较,差异无统计学意义(P>0.05);研究组术中出血量少于对照组,住院时间短于对照组,差异均有统计学意义(P<0.05);研究组术后1、2、3 d的视觉模拟评分均低于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率为94.44%(34/36),高于对照组的75.00%(27/36),差异有统计学意义(P<0.05);研究组术后并发症发生率为11.11%(4/36),低于对照组的36.11%(13/36),差异有统计学意义(P<0.05)。结论:PPH联合外痔切剥术治疗环状混合痔患者可减少术中出血量,缩短住院时间,降低疼痛评分和术后并发症发生率,提高治疗总有效率,效果优于外痔切剥术治疗。Objective:To observe effects of procedure for prolapse and hemorrhoids(PPH)and external hemorrhoidectomy in treatment of circular mixed hemorrhoids.Methods:72 patients with circular mixed hemorrhoids were selected as the research objects,and were divided into control group and study group according to the systematic sampling method,36 cases in each group.The control group was treated with Milligan-Morgan method,while the study group was treated with PPH combined with external hemorrhoidectomy.The levels of surgical related indexes,the postoperative pain degree score,the clinical efficacy and the postoperative complication rate were compared between the two groups.Results:There was no statistically significant difference in the operation time between the two groups(P>0.05).The amount of intraoperative blood loss in the study group was less than that in the control group;the hospitalization time was shorter than that in the control group;and the differences were statistically significant(P<0.05).The visual analog scores of the study group were lower than those of the control group 1,2,and 3 days after the surgery,and the differences were statistically significant(P<0.05).The effective rate of treatment in the study group was 94.44%(34/36),which was higher than 75.00%(27/36)in the control group,and the difference was statistically significant(P<0.05).Further,the postoperative complication rate in the study group was 11.11%(4/36),which was lower than 36.11%(13/36)in the control group,and the difference was statistically significant(P<0.05).Conclusions:PPH combined with external hemorrhoidectomy in the treatment of circular mixed hemorrhoids can reduce the intraoperative blood loss,shorten the hospitalization time,reduce the pain score and the postoperative complication rate,and improve the treatment effective rate.Moreover,it is superior to Milligan-Morgan treatment.
