摘　　要：肌少症具有发病率高和致死率高的特点,严重影响患者的生活质量。延续性护理作为近年来新兴的护理模式,旨在提高患者和家属的自我护理能力,可有效减少患者的再住院率,改善患者生活质量。该文从心理干预、营养指导、患者运动和用药指导几个方面入手,探讨了延续性护理在终末期肾病合并肌少症患者中的应用,对延续性护理在终末期肾病患者中的应用研究进行分析,并描述了延续性护理的实施内容及形式,旨在助其在临床上进一步推广。Sarcopenia has the characteristics of high morbidity and mortality, which seriously affects the quality of life of patients. Continuity of care, as an emerging nursing model in recent years, aims to improve self-care abilities of patients and their families, which can effectively reduce the rate of rehospitalization, and improve the quality of life of patients. This article starts with psychological intervention, nutrition guidance, patient exercise and medication guidance,and explores the application of continuity of care in patients with end-stage renal disease complicated with sarcopenia,analyzes the application of continuity of care in patients with end-stage renal diseases, and describes the implementation content and form of continuity of care, aiming to help its further promotion in clinic.
