摘 要:目的研究旋前方肌对前臂旋转功能的影响。方法选取2017年至2020年我们手术治疗的重度肘管综合征患者,随机分为两组,A组常规做肘部尺神经松解前置术;B组做肘部尺神经松解前置术的同时加做腕部旋前方肌支配神经切断转位尺神经手术。随访比较两组患者术前术后前臂旋前、旋后角度,中立位时旋前、旋后最大静止肌力及上肢功能(disability of arm-shoulder-hand,DASH)评分。结果两组患者术前术后组内比较前臂旋前、旋后角度及旋后肌力差异均无统计学意义(P>0.05);旋前肌力及DASH评分差异有统计学意义(P<0.05),且术前均大于术后。两组间比较术前差异均无统计学意义(P>0.05);术后两组间前臂旋前、旋后角度、旋后肌力及DASH评分差异均无统计学意义(P>0.05);术后两组间旋前肌力差异有统计学意义(P<0.05),且A组大于B组。结论旋前方肌能增加前臂旋前的力量,但对前臂旋转角度及上肢功能无明显影响。Objective To study the effect of pronator quadratus muscles on pronation function of forearm.Methods The patients with severe cubital tunnel syndrome treated by operation from 2017 to 2020 were selected and randomly divided into two groups.The patients of group A underwent routine ulnar nerve release and anterior transposition at the elbow;those of group B underwent ulnar nerve release and anterior transposition,and at the same lime the nerve innervating pronator quadratus muscle was cut and transferred to the ulnar nerve at the wrist.The preoperative and postoperative pronation and supination angles of forearm,the maximum static muscle strength of pronation and supination in neutral position and the disability of arm shoulder hand(DASH)score were compared between the two groups.Results There was no significant difference in pronation angle,supination angle and supination muscle strength before and after operation of both groups(P>0.05).There were statistically significant differences in pronation muscle strength and DASH score before and after operation of both groups(P<0.05),and the pronation muscle strength before operation was greater than that after operation.There was no significant difference between the two groups before operation(P>0.05).There was no significant difference in forearm pronation angle,supination angle,supination muscle strength and DASH score between the two groups after operation(P>0.05).The difference of pronation muscle strength between the two groups was statistically significant(P<0.05),and that of group A was greater than that of group B.Conclusion The pronator quadratus muscle can increase the pronation strength of forearm,but it has no significant effect on the rotation angle of forearm and the function of upper limb.
