摘　　要：目的探讨采用游离显微削薄股前外侧穿支皮辦(anterolateral thigh perforator flap,ALTP)修复前臂残端创面的临床效果。方法自2015年1月至2018年12月,我们对8例前臂离断毁损伤残端创面采用削薄ALTP修复术中在显微镜视下根据筋膜层血管网走行及受区需要削薄皮瓣,术后早期开始残肢功能锻炼:结果术后本组8例皮瓣均存活,皮瓣切取面积为6.0 cm×5.0 cm〜9.0 cm×7.0 cm。术后所有患者均获得随访,时间为12〜24个月,平均15个月。所有皮瓣外形不臃肿,质地柔软,皮瓣感觉功能恢复好残端安装假肢适配良好结论采用削薄ALTP修复前臂残端创面,可保留更多前臂长度,皮瓣外形美观,质地耐磨,具备感觉,是一种较好的修复方式。Objective To explore the clinical efficacy of free micro-thin anterolateral thigh perforator flap(ALTP)for repair of forearm stump wound.Methods From January 2015 to December 2018,8 cases of forearm amputated wound were repaired wilh thin ALTP.During the operation,the flap was thinned according to the course of the fascial vascular network and the need of the recipient area under the microscope,and the functional exercise of the residual limb was started early after the operation.Results All the 8 flaps survived.Hie area of the flaps ranged from 6.0 cm×5.0 cm to 9.0 cm×7.0 cm.All the patients were follow-up for 12 to 24 months with an average of 15 months.All the flaps were soft and had good sensory function.Hie prosthesis fitted well.Conclusion The application of thin ALTP to repair the wounds of forearm stump can keep more forearm length.The skin flap has beautiful appearance,wear-resistant texture and good feeling.It is a good repair medthod.
