龙胆泻肝汤加味治疗尿路感染的临床疗效及对T淋巴细胞亚群和炎症因子的影响

摘　　要：目的观察龙胆泻肝汤加味治疗尿路感染的临床疗效及对T淋巴细胞亚群和炎症因子的影响。方法将120例尿路感染患者按照随机数字表法分为2组,对照组60例予宁泌泰胶囊口服治疗,治疗组60例予龙胆泻肝汤加味治疗。2组均治疗7 d为1个疗程,下尿路感染治疗1个疗程,上尿路感染治疗2个疗程。比较2组疗效、主要症状消失时间;比较2组治疗前后T淋巴细胞亚群表达水平及炎症因子变化;比较2组随访3个月复发率。结果治疗组总有效率98.33%(59/60),对照组总有效率93.33%(56/60),治疗组疗效优于对照组(P<0.05)。治疗组尿频、尿急、小便涩痛、血尿及下腹疼痛消失时间较对照组缩短(P<0.05)。2组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较本组治疗前升高(P<0.05),CD8^(+)较本组治疗前降低(P<0.05),且治疗后治疗组CD3^(+)、CD4、CD4^(+)/CD8^(+)均高于对照组(P<0.05),CD8^(+)低于对照组(P<0.05)。2组治疗后白细胞介素6(IL-6)、IL-8及肿瘤坏死因子α(TNF-α)均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。治疗组复发率3.33%(1/30),对照组复发率32.00%(8/25),治疗组远期疗效优于对照组(P<0.05)。结论龙胆泻肝汤加味治疗尿路感染,可有效改善患者临床症状,调节T淋巴细胞亚群表达水平,降低炎症因子水平,且不易复发。ObjectiveTo observe the clinical effect ofmodified Longdan Xiegan Decoction for urinary tract infection(UTI)and its effect on T lymphocyte subsets and inflammatory factors.MethodsTotally 120 UTI patients were randomly divided into control group(n=60,given oral Ningmitai Capsule)and treatment group(n=60,given modified Longdan Xiegan Decoction),7 days for a course of treatment,one course of treatment for patients with lower UTI,and two course of treatment for patients with upper UTI.Clinical curative effect,timing of the disappearance of the main symptoms,expression of T lymphocyte subsets and inflammatory factors,and recurrence rate during the 3-month follow-up between groups were compared.ResultsThe total effective rates were significantly higher in treatment group than in control group[98.33%(59/60)vs 93.33%(56/60),(P<0.05),respectively].Compared with control group,treatment group had shorter duration of urinary frequency,urgency,painful urination,hematuria and disappearance time of lower abdominal pain(P<0.05).Compared with before treatment,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)after treatment were higher in groups(P<0.05),which in treatment group were significantly increased(P<0.05);CD8^(+)after treatment in groups was lower(P<0.05),which in treatment group was significantly decreased(P<0.05);IL-6,IL-8 and TNF-αafter treatment in groups were lower(P<0.05),which in treatment group were significantly decreased(P<0.05).Recurrence rate was lower in treatment group than in control group[3.33%(1/30)vs 32.00%(8/25),(P<0.05),respectively],and long-term effect in treatment group was better.ConclusionForUTI patients,modified Longdan Xiegan Decoction has a significant clinical effect,can regulate expression of T lymphocyte subsets,reduce inflammatory factors,and which is not easy to relapse.
