温针灸、药线点灸联合乳癖汤治疗乳腺增生症气滞血瘀型的疗效及对患者性激素、血脂水平的影响

摘　　要：目的观察温针灸、药线点灸联合乳癖汤治疗乳腺增生症气滞血瘀型临床疗效及对患者性激素、血脂水平的影响。方法将120例乳腺增生症气滞血瘀型患者按照随机数字表法分为2组,对照组60例予温针灸、药线点灸治疗,治疗组60例在对照组治疗基础上联合乳癖汤口服治疗。2组均月经来潮结束至下一月经来潮前为1个疗程,治疗3个疗程。比较2组疗效;观察2组治疗前后性激素雌二醇(E_(2))、黄体生成激素(LH)、卵泡刺激素(FSH)及泌乳激素(PRL)水平变化;观察2组治疗前后疼痛视觉模拟评分(VAS)、症状自评量表(SCL-90)评分、中医证候评分变化;比较2组治疗前后血脂指标总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)水平变化;记录2组治疗中不良反应发生情况。结果治疗组总有效率91.67%(55/60),对照组总有效率50.00%(30/60),治疗组疗效优于对照组(P<0.05)。2组治疗后血清E_(2)、LH、FSH、PRL水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后疼痛VAS、SCL-90评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后各项中医证候评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后TG、LDL-C水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗前后TC、HDL-C水平比较差异无统计学意义(P>0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论温针灸、药线点灸联合乳癖汤治疗乳腺增生症气滞血瘀型,能改善患者乳房疼痛症状及心理状态,降低患者性激素水平,改善血脂水平。Objective To investigate the curative effect of warm acupuncture(WA),medicated thread moxibustion(MTM)combined with Rupi Decoction on hyperplasia of mammary glands of qi and blood stasis and its influence on sex hormone and blood lipid indexes.MethodsTotally 120 patients with hyperplasia of mammary glands of qi and blood stasis were randomly divided into control group(n=60)and treatment group(n=60).Patients in control group were treated with WA and MTM,patients in treatment group were additionally treated with Rupi Decoction on the basis of control group.A course of treatment was form end of menstrual onset to next menstrual onset,treated for 3 courses.Curative effect,estradiol(E_(2)),luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL),pain visual analogue score(VAS),Symptom Checklist-90(SCL-90)score,traditional Chinese medicine(TCM)syndrome score,blood lipid indexes including total cholesterol(TC),triglyceride(TG),high density lipoprotein-cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C)between groups were compared before and after treatment.adverse reaction between groups was recorded.ResultsThe total effective rates were better in treatment group than in control group[91.67%(55/60)vs 50.00%(30/60),(P<0.05),respectively].Compared with before treatment,serum E_(2),LH,FSH,PRL,VAS in groups were lower after treatment(P<0.05),which in treatment group were significantly decreased(P<0.05);decreased SCL-90 score,TCM syndrome scores in groups were discovered(P<0.05),and which in treatment group were significantly decreased(P<0.05);and decreased total scores,TG,LDL-C in groups were discovered after treatment(P<0.05),which in treatment group were significantly decreased(P<0.05);and difference was no significant in TC and HDL-C between groups before and after treatment(P>0.05);difference was no statistically significant in adverse reactions between groups(P>0.05).ConclusionFor patients with hyperplasia of mammary glands of qi blood stasis,warm acupuncture,MTM combined with Rupi Decoction c
