肌骨超声引导刃针松解治疗神经根型颈椎病临床研究

摘　　要：目的观察肌骨超声引导刃针松解治疗神经根型颈椎病的临床疗效。方法将120例神经根型颈椎病患者按照随机数字表法分为2组,治疗组60例予肌骨超声引导刃针松解治疗,对照组60例予非直视下刃针松解治疗。2组均隔3 d治疗1次,治疗5次。比较2组疗效;比较2组治疗前后田中靖久颈椎病症状量表(YT20)评分;观察2组治疗安全性及疗程结束后6个月随访时复发率。结果治疗组脱落5例,实际完成55例,对照组脱落7例,实际完成53例。治疗组治愈率63.6%(35/55)、总有效率90.9%(50/55),对照组治愈率41.5%(22/53)、总有效率86.8%(46/53),治疗组疗效优于对照组(P<0.05)。2组治疗后YT20评分均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。治疗组不良反应发生率1.8%(1/55),对照组不良反应发生率11.3%(6/53),治疗组低于对照组(P<0.05)。治疗组复发率5.7%(2/35),对照组复发率13.6%(3/22),治疗组复发率低于对照组(P<0.05)。结论肌骨超声引导刃针松解治疗神经根型颈椎病,可缓解患者临床症状,提高治愈率,降低复发率。Objective To observe clinical study of musculoskeletal ultrasound-guided knife-needle lysis for cervical spondylotic radiculopathy(CSR).Methods Totally 120 CSR patients were randomly divided into two groups,60 cases in each group.Patients in treatment group were treated with musculoskeletal ultrasound-guided knife-needle lysis,and patients in control group were treated with same therapyexcept guided by non-line-of-sight.Both groups were treated 5 times,once every 3 days.Curative effect and 20-point scale of CSR developed by Japanese scholar,Tanaka Yasuhisa,(YT20)were compared between groups before and after treatment.Treatment safety and recurrence rate at 6-month follow-up after treatment were observed.Result There were 5 cases and 7 cases for withdrew out,and 55 cases and 53 cases for actual completion in treatment group and in control group,respectively.The cure rate and total effective rates in treatment group were better than those in control group[63.6%(35/55)vs 41.5%(22/53),90.9%(50/55)vs 86.8%(46/53),(P<0.05),respectively].Compared with before treatment,YT20 score in groups was higher after treatment(P<0.05),which in treatment group was significantly increased(P<0.05);adverse reactions and recurrence were lower in treatment group than in control group[1.8%(1/55)vs 11.3%(6/53),5.7%(2/35)vs 13.6%(3/22),(P<0.05),respectively].Conclusion For CSR patients,musculoskeletal ultrasound-guided knife-needle lysis can relieve symptoms,improve cure,and reduce recurrence.
