单纯前后位透视下胸椎经皮椎弓根螺钉植入的可行性研究

摘　　要：目的分析单纯前后位透视下胸椎经皮椎弓根螺钉植入的可行性。方法通过计算机系统随机选择2019年1月至2020年12月大连市普兰店区中心医院收治的180例需行下胸椎经皮椎弓根螺钉植入术治疗的患者为研究对象,将其随机分为A组(90例,采用传统经皮椎弓根螺钉植入治疗)和B组(90例,采用C型臂X线机单纯前后位透视下胸椎经皮椎弓根螺钉植入治疗),统计对比两组患者术中的透视次数、手术相关指标及螺钉置入优良率。结果B组患者术中透视次数、术中出血量、住院期间引流量少于A组患者,手术时间、住院时间均显著短于A组患者(P<0.05);B组螺钉置入优良率显著高于A组(P<0.05)。结论在单纯前后位透视下行胸椎经皮椎弓根螺钉植入术,可有效提高螺钉置入的优良性,减少手术透视次数,对提高手术安全、促进患者康复均具有积极的意义。Objective To analyze the feasibility of percutaneous pedicle screw implantation in thoracic vertebra under simple anteroposterior fluoroscopy.Methods A total of 180 patients who needed percutaneous pedicle screw implantation of lower thoracic vertebrae admitted to our hospital from January 2019 to December 2020 were randomly selected as the subjects of this study.The selected subjects were randomly divided into group A(90 cases,received traditional percutaneous pedicle screw implantation)and group B(90 cases,received C-arm X-ray machine with simple anterior and posterior fluoroscopy for thoracic pedicle screw implantation).The number of intraoperative fluoroscopy,surgical indexes and the excellent and good rate of screw placement were statistically compared between the two groups.Results The number of intraoperative fluoroscopy,intraoperative blood loss,and drainage volume during the hospital stay in group B were less than those in group A,and the operation time and hospitalization time were significantly shorter than those in group A(P<0.05);the excellent and good rate of screw placement in group B was significantly higher than that in group A(P<0.05).Conclusion Thoracic percutaneous pedicle screw implantation under simple anteroposterior fluoroscopy can effectively improve the optimal screw placement and reduce the number of surgical fluoroscopy,which has positive significance for improving surgical safety and promoting patient rehabilitation.
