果酸化学剥脱治疗面部痤疮患者的效果

摘　　要：目的观察羟基乙酸(果酸)化学剥脱治疗面部痤疮患者的疗效。方法选择2018年12月1日至2019年5月31日于北京友谊医院皮肤科就诊的36例面部痤疮患者,男4例、女32例,年龄24~36(28.06±2.88)岁。应用浓度20%果酸治疗,评估治疗前后患者痤疮分级、痤疮数目、痤疮消退率、治疗有效率、毛孔改善情况及患者满意度。结果治疗后与治疗前相比,各指标评价均有所改善。果酸治疗前,患者痤疮Pillsbury分级为Ⅰ级9例、Ⅱ级18例、Ⅲ级9例,治疗后Ⅰ级28例、Ⅱ级8例、Ⅲ级0例,治疗前后比较,差异有统计学意义(χ2=22.603,P<0.001)。治疗前患者痤疮数38.64±15.57,治疗后患者痤疮数16.17±11.49,差异有统计学意义(t=6.967,P<0.001)。治疗后,36例患者中痊愈2例,显效17例,有效11例,无效2例,恶化4例,有效率83.33%。果酸治疗前患者皮肤毛孔值1263.67±593.44,治疗后降至1196.33±579.27,治疗前后比较,差异有统计学意义(t=3.155,P<0.05)。治疗结束后,患者均对疗效满意。结论果酸治疗痤疮患者是一种较为安全有效的方法,值得临床应用。Objective To study the clinical effect of alpha hydroxy acids on facial acne patients.Methods A total of 36 patients with facial acne who were treated at the Department of Dermatology,Beijing Friendship Hospital from December 1,2018 to May 31,2019 were selected.All patients were treated with 20%alpha hydroxy acids.Treatment efficiency,lesions improvement and patients'satisfaction were evaluated.Results All evaluation indexes were improved after treatment.Before treatment,patients with acne were classified into gradeⅠ(9 cases),gradeⅡ(18 cases),and gradeⅢ(9 cases)through Pillsbury Grading,and after treatment,there were 28 cases of gradeⅠ,28 cases of gradeⅡ,and 0 cases of gradeⅢ,which had statistic differences(χ2=22.603,P<0.001).The number of lesions before treatment was 38.64±15.57,and that was 16.17±11.49 after treatment,and the difference was statistically significant(t=6.967,P<0.001).After treatment,2 cases were cured,17 cases were markedly effective,11 cases were effective,2 cases were ineffective and 4 cases were worsening.30 patients were cured,markedly effective or effective,and the total effective rate was 83.33%.The value of skin pores before treatment was 1263.67±593.44,and decreased to 1196.33±579.27 after treatment.The difference was statistically significant(t=3.155,P<0.05).After the treatment,all the patients were satisfied with the effect.Conclusions Alpha hydroxy acid is a safe and effective method for acne patients.
