补肾安胎饮加减联合穴位贴敷治疗先兆流产疗效及对患者血清β-HCG、孕酮、雌二醇的影响

摘　　要：目的:研究补肾安胎饮加减联合穴位贴敷治疗先兆流产的临床疗效及对血清β-绒毛膜促性腺激素(β-HCG)、孕酮(P)、雌二醇(E_(2))的影响。方法:以随机数字表法将110例先兆流产脾肾两虚证患者分为联合组(56例)和对照组(54例)。联合组给予补肾安胎饮加减联合穴位贴敷及西药治疗,对照组给予西药治疗。比较两组患者治疗前后子宫螺旋动脉血流动力学[收缩期峰值血流(PSV)、舒张末期流速(EDV)和阻力指数(RI)]、炎症因子[γ-干扰素(INF-γ)、白细胞介素-2(IL-2)、IL-4]和血清β-HCG、P、E_(2)水平及中医证候积分变化,评估疗效。结果:联合组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,联合组患者PSV、EDV水平和血清IL-4、β-HCG、P、E_(2)水平高于对照组,RI、INF-γ、IL-2水平和中医证候积分低于对照组(均P<0.05)。结论:补肾安胎饮加减联合穴位贴敷及西药治疗先兆流产脾肾两虚证,可调节炎症因子、子宫螺旋动脉血流动力学水平,明显改善血清β-HCG、P、E_(2)水平及患者症状。Objective:The purpose of the study was to study the clinical effect of modified Bushen Antai decoction combined with acupoint application in adjuvant treatment of threatened abortion,and the influence on serumβ-human chorionic gonadotropin(β-HCG),progesterone(P)and estradiol(E_(2)).Methods:110 patients with acupoint application of Pishen Liangxu syndrome were divided into combination group(56 cases)and control group(54 cases)by random number table method.Patients in the combination group were treated with modified Bushen Antai decoction combined with acupoint application based on western medicine treatment,while the control group were treated with western medicine.Uterine spiral artery hemodynamics[peak systolic velocity(PSV),end-diastolic velocity(EDV)and resistance index(RI)],inflammatory factors[interferon-γ(INF-γ),interleukin-2(IL-2)and IL-4],serumβ-HCG,P,E_(2) levels and TCM syndrome scores were compared between the two groups.The curative effect was evaluated.Results:Total effective rate in the combination group was significantly higher than that in the control group(P<0.05).After treatment,PSV,EDV,serum IL-4,β-HCG,P and E_(2) levels in the combination group were significantly increased,and higher than those in the control group.RI,INF-γ,IL-2 and TCM syndrome scores were significantly decreased,and lower than those in the control group(all P<0.05).Conclusion:Applying modified Bushen Antai decoction combined with acupoint application based on western medicine to treat patients with acupoint application of Pishen Liangxu syndrome can regulate levels of inflammatory factors and uterine spiral artery hemodynamicsand,significantly improve levels of serumβ-HCG,P and E_(2),thus promoting the improvement of symptoms.
