清热止咳方治疗小儿支气管炎疗效及对患儿血气指标及细胞因子的影响

摘　　要：目的:研究清热止咳方治疗支气管炎疗效及对患儿血气指标、细胞因子、免疫的影响。方法:按照随机数字表法将106例支气管炎患儿分为对照组51例和治疗组55例,对照组给予常规西药治疗,治疗组在对照组基础上给予清热止咳方治疗。比较两组患儿治疗前后中医症状积分、血气指标、细胞因子、免疫功能,并评估疗效和安全性。结果:治疗组总有效率94.55%高于对照组的82.35%(P<0.05)。治疗后,治疗组中医证候积分各项低于对照组(均P<0.05)。治疗后治疗组动脉血二氧化碳分压(PaCO_(2))水平低于对照组,而pH、PaO_(2)高于对照组(均P<0.05);治疗组干扰素γ(IFN-γ)水平显著高于对照组,SP-D、TNF-α、IL-17水平均低于对照组(均P<0.05)。治疗后治疗组IgA、IgG、IgM水平均高于对照组(均P<0.05)。两组总不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清热止咳方治疗支气管炎可改善临床症状及血气指标,降低炎症反应,改善免疫功能,疗效较好。Objective:To study Qingre Zhike prescription on clinical effect,blood gas indexes and cytokines in children bronchitis.Methods:According to random number table method,106 children with bronchitis were divided into control group(51 cases)and treatment group(55 cases).The control group were treated with conventional western medicine,and the treatment group additionally treated with Qingre Zhike prescription.Changes in TCM symptom scores,blood gas indexe and cytokine,immune function before and after treatment were compared between the two groups.Curative effect and safety were evaluated.Results:Total efficiency rate in the treatment group 94.55%was significantly higher than that in the control group 82.35%(P<0.05).After treatment,TCM symptom scores in the treatment group were lower than the control group(all P<0.05).The partial pressure of carbon dioxide(PaCO_(2))in the treatment group was lower than that in the control group,but pH,PaO_(2) were higher than those in the control group(all P<0.05).The level of interferon gamma(IFN-γ)in the treatment group was higher than that in the control group,but SP-D,TNF-α,IL-17 were lower than those in the control group(all P<0.05).The levels of IgA,IgG,IgM in the treatment group were higher than those in the control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Qingre Zhike prescription can improve clinical symptoms,blood gas index,and improve respiratory function,immune function and better clinical effect.
