炙甘草汤加减联合依那普利治疗慢性阻塞性肺疾病合并心力衰竭疗效研究

摘　　要：目的:探讨炙甘草汤加减联合依那普利治疗慢性阻塞性肺疾病(COPD)合并心力衰竭(HF)疗效。方法:选取73例COPD合并HF患者为观察对象,随机分为两组。两组均给予常规治疗,在此基础上对照组口服依那普利,中药组在对照组基础上联合炙甘草汤加减进行治疗。比较两组患者治疗前后的临床疗效、中医证候积分、心肺功能及血清生化指标水平。结果:治疗后,中药组患者总有效率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后各中医证候积分、总分和血清生化指标较治疗前降低(均P<0.05)。患者心肺功能指标较治疗前改善,且治疗后上述指标组间比较,差异有统计学意义(均P<0.05)。两组患者在治疗过程中均未发生明显不良反应。结论:炙甘草汤加减联合依那普利治疗COPD合并HF患者,临床疗效较好,可改善患者中医证候、心肌损伤和肺功能,安全性较好。Objective:To explore efficacy of modified Zhigancao decoction combined with enalapril in treatment of chronic obstructive pulmonary disease(COPD)with heart failure(HF).Methods:73 patients with COPD complicated with HF were selected as observation subjects,and randomly divided into two groups.Patients in two groups were given conventional treatment,the control group was given oral administration of enalapril on the basis,the traditional Chinese medicine group was combined with modified Zhigancao decoction on basis of the control group.The clinical efficacy,TCM syndromes scores,cardio-pulmonary function and levels of serum biochemical indicators were compared between the two groups before and after treatment.Results:After treatment,the total effective rate of the traditional Chinese medicine group was significant hingher than that in the control group(P<0.05).The scores of various syndromes and total score and levels of serum biochemical indicators of the two groups were significantly decreased compared with those before treatment(all P<0.05).The cardio-pulmonary function indicators of patients were significantly increased compared with those before treatment.There were significant differences in the above-mentioned indicators between the groups after treatment(all P<0.05).There were no adverse reactions in the two groups during treatment.Conclusion:Modified Zhigancao decoction combined with enalapril has significant clinical efficacy in the treatment of patients with COPD and HF.It can improve traditional Chinese medicine symptoms,myocardial injury and lung function.Its has better safely.
