血清性激素、5-LOX、LXA4检测在子宫内膜异位症不孕患者中的临床价值研究

摘　　要：目的探讨血清性激素、5-脂氧合酶(5-LOX)、脂氧素A4(LXA4)检测在子宫内膜异位症(EM)不孕患者中的临床价值。方法选取我院2018年6月至2020年6月收治的136例EM不孕患者为观察组2,另选取同期55例EM非不孕患者为观察组1以及健康体检女性40例为对照组。比较三组血清性激素、5-LOX及LXA4水平。136例EM不孕患者参照美国生殖协会分期标准(r-AFS)分为Ⅰ期组36例、Ⅱ期组49例、Ⅲ期组31例、Ⅳ期组20例。比较不同分期EM不孕患者血清性激素、5-LOX及LXA4水平。采用Spearman相关性分析血清性激素、5-LOX及LXA4水平与分期的关系。采用ROC曲线分析血清性激素、5-LOX及LXA4水平诊断EM不孕的价值。结果观察组2血清FSH、LH、E 2水平明显高于对照组和观察组1(P<0.05),血清LXA4、5-LOX水平明显低于对照组和观察组1(P<0.05)。Ⅰ期组、Ⅱ期组、Ⅲ期组、Ⅳ期组EM不孕患者血清FSH、LH、E 2水平呈逐渐升高趋势,差异有统计学意义(P<0.05),血清LXA4、5-LOX水平呈逐渐降低趋势,差异有统计学意义(P<0.05);EM不孕患者r-AFS分期与血清FSH、LH、E 2水平呈正相关(r=0.583、0.731、0.649,P<0.05),与血清LXA4、5-LOX水平呈负相关(r=-0.702、-0.619,P<0.05);经ROC曲线分析,五项指标联合诊断EM不孕的AUC为0.870。结论子宫内膜异位症不孕患者病情严重程度与血清性激素、5-LOX及LXA4水平具有显著相关性,为临床治疗EM不孕提供了另一个思考方向。Objective To investigate the clinical value of serum sex hormones,5-lipoxygenase(5-LOX)and lipoxin A4(LXA4)in the diagnosis of endometriosis(EM)-associated infertility.Methods 136 cases of EM infertile patients in our hospital from June,2018 to June,2020 were selected as the observation group 2,while 55 cases of EM non-infertile patients during the same period were selected as the observation group 1,and 40 cases of healthy women were selected as the control group.Serum sex hormone,5-LOX and LXA4 levels were compared among these three groups.The observation group was further divided into four subgroups according to the classification standards of revised American Fertility Society(r-AFS)as following:stageⅠgroup(n=36),stageⅡgroup(n=49),stageⅢgroup(n=31)and stageⅣgroup(n=20).Serum levels of sex hormones,5-LOX and LXA4 were compared within groups and subgroups.Spearman correlation was applied to analyze the relationship between serum sex hormones,5-LOX and LXA4 levels and r-AFS stages.ROC curve was drawn to evaluate the value of serum sex hormones,5-LOX and LXA4 levels in the diagnosis of EM-associated infertility.Results Serum levels of FSH,LH and E 2 in the observation group 2 were significantly higher than those in the control group and observation group 1(P<0.05),while serum levels of LxA4 and 5-LOX in the observation group 2 were significantly lower than those in the control group and observation group 1(P<0.05).With the increase of r-AFS stage,serum FSH,LH and E 2 levels showed an increasing trend(P<0.05),while serum LXA4 and 5-LOX levels showed a decreasing trend(P<0.05).The r-AFS staging of EM-associated infertility patients was positively correlated with serum FSH,LH,and E 2 levels(r=0.583,0.731,0.649,P<0.05)while negatively correlated with serum LXA4 and 5-LOX levels(r=-0.702,-0.619,P<0.05).Area under the ROC curve(AUC)of combined detection of five indicators in the diagnosis of EM-associated infertility was 0.870.Conclusion There is a significant correlation between the severity of endometriosis i
